Job Description:
We are seeking a highly skilled and motivated Infusion Pharmacist to join us. As an Infusion Pharmacist, you will be responsible for ensuring the safe and effective administration of medications through infusion therapy. This position offers the opportunity to work in a collaborative and patient-focused environment, contributing to the delivery of high-quality healthcare services.
Facility Type : Closed Door Home Infusion Pharmacy
Job Type/Standard Shift:
• Day shift/Full-time/8 hours
• Monday - Friday
• Rotate On-Call
Responsibilities:
1. Accurately prepare and compound medications for infusion therapy based on physician orders.
2. Conduct quality assurance checks to ensure the integrity and sterility of prepared infusions.
3. Collaborate with healthcare professionals to develop and implement appropriate treatment plans.
4. Provide patient education on the proper administration of infusion therapies, including potential side effects and monitoring parameters.
5. Monitor patients receiving infusion therapy for adverse reactions and collaborate with the healthcare team to adjust treatment plans as needed.
6. Maintain accurate and up-to-date records of medication orders, compounding activities, and patient responses.
7. Ensure compliance with state and federal regulations governing pharmacy practice and adhere to safety standards.

Experience:
• MUST have Sterile Compounding experience
• MUST have Specialty Infusion
• MUST have previous Infusion experience
License/Certification:
• Must have a Virginia Pharmacy License
