Crossroads is a leading addiction treatment provider of outpatient medication-assisted treatment (MAT). We treat patients with opioid use disorder (OUD) using medications such as methadone and suboxone / buprenorphine.

Assist in developing and monitoring recovery plans in collaboration with patients
Teach, role model, and coach patients on how to access helpful resources within their community and healthcare systems, how to cultivate best practices for management of symptoms related to their treatment, and assist with developing necessary skills to attain and maintain employment / housing.
Work with patients to identify how they are reacting to prescribed treatment medications and communicate any impactful changes with any necessary medical providers related to the patients treatment.
At minimum, High School Diploma or GED required.
Must hold an active, unencumbered, Certified Peer Support Specialist certification.
Experience in healthcare, addiction medicine, community health, care coordination preferred.
Expected hours for this role are 37-40 hours per week with few opportunities for overtime.
Typical work hours are flexible, but traditional office hours are preferred, 8 : 00 AM - 5 : 00 PM.
