We are searching for a dynamic individual to become the RE-ENTRY CARE COORDINATOR for our Southern Colorado location.
SUMMARY:
Provides reentry services and care coordination for participants leaving jail or prison and entering a functional role in society.
All positions require pre-employment drug screening and background checks.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Include the following. Other duties may be assigned.
Responsible for developing care planning, patient needs assessments, and resource linkages.
Facilitates psychoeducation groups by the CTP policies and procedures.
Acts as liaison, establishing networks and collaboration with hospitals, community health centers, and state and human service agencies about participant services and needs.
Document admissions, care management needs, transportation notes, discharge, and discharge planning.
Set up participant transportation to and from appointments (medical, psychiatric, court hearings, etc.).
Administer case management assessments to identify participant needs and ensure the Addiction Severity Index (ASI) is completed at intake.
Organize a schedule of participant bed dates, transitioning levels of care, and consistent communication with the Medicaid Placement Coordinators.
Assists treatment team with participant intake, transfer, and discharge assessments.
When applicable, collaborate with the facility treatment team (medical, psychiatric, and SUD) to identify patients' needs.
Refer participants to Intensive Case Management (ICM) to help meet the patient’s basic needs (i.e., clothing, medical, judicial, religious, etc.) from available resources.
SUPERVISORY RESPONSIBILITIES: N/A
QUALIFICATIONS:
Education:
Associate degree in a behavioral health-related field and ability to obtain CAT certification within 12 months;
OR
Actively enrolled in an undergraduate program in a behavioral health-related field with a current CAT.
Experience:
Experience working in the field of alcohol, tobacco, and drug education/recovery as well as with other high-risk populations.
Other:
Must have a good understanding of substance use, mental health, and treatment delivery systems.
Advanced computer skills are required.
CPR and first-aid certification. (Initial training and renewals paid for by agency).
Must be able to take vital signs, recognize possible medical problems, and refer to the medical staff when necessary.
Valid driver's license and good driving record (must provide a current unofficial DMV driving record upon hire).
Able to communicate effectively in a multidimensional treatment program.
