Constellation Health is seeking a Full-Time Physical Therapist to work in the Orange/New Haven area. 

Job Description

We are looking for a Physical Therapist - PT for a full-time position who can travel throughout an assigned territory and act as the patient’s partner throughout the trip of restoring motion and ensuring that the patient will function at their personal best. You will maximize the quality of life and movement potential.

Responsibilities: 

• Initiates, reviews, and implements the PT plan of care for each patient based on the initial assessment and prescribed plan of care.
• Educate patients and caregivers on the disease process, medications, plan of care and individualized treatment plan.
• Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system. Submits all clinical documentation, time records and other information in the required time frame.

Qualifications: 

• Licensed Physical Therapist in the state of Connecticut with at least 1 year of clinical experience; Home care preferred
• New graduates are encouraged to apply, we provide excellent support and guidance to get your career started on a path to success.
• Home Health Oasis experience is a plus. 
• Have reliable transportation and be willing to cover an assigned territory.
