We are seeking a compassionate and dedicated Licensed Practical Nurse to join our Home Health team. As a LPN - Home Health Nurse, you will provide high-quality nursing care to patients in the comfort of their own homes. You will play a critical role in helping patients maintain their independence and improve their overall well-being.

Responsibilities:

-Follow plan of care to care for patient in a residential or group home setting
- Administer medications and treatments per plan of care and mar
- Utilize aseptic technique to prevent infection during procedures
- Ensure compliance with documentation guidelines
- Coordinate care with other nurses on case and management as needed
- Adhere to infection control protocols to maintain a safe environment for patients

-conduct audits, assist with staffing, training, and other tasks when in office

Qualifications:
- Valid LPN license in the state of Virginia or Compact state license
- Minimum of 2 years of nursing experience, preferably in home health or a related field
- Proficient in using electronic medical record systems
- Strong communication and interpersonal skills
- Ability to work independently and prioritize tasks effectively
