ABOUT LIFE PITTSBURGH -
LIFE Pittsburgh, a growing nonprofit organization with approximately 400 employees, is a community-based alternative to nursing home care and assisted living. Through the LIFE Pittsburgh program, frail older adults can remain in their own home and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community. Our health teams of geriatric physicians and nurse practitioners, nurses, social workers, rehabilitation specialists and other health care professionals assess our Participants' needs and desires, plan and approve services with them and their family or caregiver, monitor for changes and provide timely interventions. Primary care and community services are provided through our Day Health Centers and through our in-home program according to an individual Participant's needs.

Immediate Need for Registered Nurse (RN)
($10,000 Sign On Bonus plus 40 extra hours PTO)
Within an Interdisciplinary Team (IDT) setting, responsible for promoting and maintaining health of LIFE Pittsburgh Participants in the community, through initial implementation of nursing care plans, providing nursing care, as well as periodic re-evaluations of Participant and family nursing needs. Conducts assessments, treatments, and provides teaching and counseling to Participants and family members.
Some essential functions of the RN position are:
•Assessments of Pre-Enrollees and Current LIFE Pittsburgh Participants
•Instruct Caregivers including training of medication mgmt., disease prevention, etc.
•Clinical Reasoning
•Provide On Call Coverage
•Thorough and Detailed Documentation
Immediate Need for Licensed Practical Nurse (LPN)
($5,000 Sign On Bonus)
Within an Interdisciplinary Team (IDT) setting, responsible for promoting and maintaining health of LIFE Pittsburgh Participants in the community, through initial implementation of nursing care plans, providing nursing care, as well as periodic re-evaluations of Participant and family nursing needs.
Some essential functions of the LPN position are:
•Administer basic personal care (wound care/dressing changes/catheters/respiratory treatments, etc)
•Monitor Participant vitals
•Report participant status and report to RN/CRNP or PCP
•Administer medication/med box preparation
•Assist with Physician order process
Requirements for RN and LPN include: a valid PA Nursing License, valid Driver's license and automobile insurance, must have access to a vehicle. Minimum of 1 year experience working with the frail and elderly population.

SHIFT -
•Primary working hours are Monday thru Friday 8:00 AM - 4:30 PM with rotating on-call shifts. Full and Part Time positions available.
LOCATION/TRAVEL REQUIRED -
•Positions based out of an Adult Day Health Center (located in North Shore, Allegheny Center, McKees Rocks and Green Tree)

• Travel to, and working in Participant residences, hospitals, nursing home facilities or other settings in which the Participant may need support.
• All business mileage is reimbursed at the Federal rate.
