• MUST HAVE COLLABORATIVE PHYSICIAN AGREEMENT • Will be administering & completing Disability Benefits Questionnaires to veterans in office • Rate is $950 for a full day, $475 for half day • Ongoing W2 contract • Training is online and paid $450
