Lemay Ave Heath & Rehab, a Columbine Health Systems facility, is located in Fort Collins, CO and is a place that focuses on the health of our residents in mind, body, and spirit.

We are accepting applications for a Registered Dietitian able to work:

• Part-time (24hrs per week) benefit eligible
• Monday through Friday
• Position is to provide assistance at 2 Columbine locations in Fort Collins and Windsor

Compensation

• Starting wage $27-$31/hr. depending on experience

Registered Dietitian

• The Dietitian is responsible for providing oversight of nutrition and diet counseling for ST Rehab patients and LTC residents.
• Performs nutrition assessments and develop nutrition care plans for residents in LTC/rehab setting.
• Plans and implements therapeutic diets.
• Collaborates with Executive Chef in the planning of menus to meet recommended nutritional requirements.
• Recommends and orders modified diets to meet the needs of each individual.
• Monitors food preparation and service procedures for compliance with food safety regulations.
• Conducts in-service training for Dietary Personnel and Nursing Staff.
• Develops written dietary policies and procedures.

Requirements

• Only credentialed Registered Dietitian's from an accredited program.
• 1 Year experience in a Skilled Nursing Facility or other healthcare preferred.
• 1 year of food service experience preferred.
• Conducts in-service training for Dietary Personnel and Nursing Staff.
• Develops written dietary policies and procedures.
• Only credentialed Registered Dietitian's considered.
• Minimum 2 years of experience, experience in a Skilled Nursing Facility or other healthcare preferred.
