POSITION SUMMARY:
The Clinical Pharmacist, Specialty Pharmacy, works under the supervision of the Site Director and/or Manager, Specialty Pharmacy Operations, and according to department policies, and procedures, and guidelines. Responsibilities include leading enrollment and ensuring ongoing prescription adherence follow-up for all patients referred by Pharmacy Liaisons for Specialty Care Management. Conducts comprehensive medication reconciliation with Electronic Medical Record (EMR) during prescription verification for each patient. Serves as the direct point of contact for pharmacy clinical support for all enrolled specialty clinics when needed with Clearway Health Specialty Pharmacy.
ESSENTIAL RESPONSIBILITIES/DUTIES:
  • Prepares or supervises Pharmacy Liaison(s) while offering support, addressing patients' questions, and conducting initial and follow up assessments for compliance per URAC accreditation.
  • Verifies all enrolled patient information.
  • Performs Specialty Pharmacy quality checks.
  • Provides consultation to customers when filling a prescription, including drug education and counseling, as outlined in any existing policies and procedures specific to the client, The Brooklyn Health Center.
  • Records notes for dispensed prescription(s) in EMR or Salesforce; covering legend, specialty, narcotics and other restricted drugs.
  • Monitors adverse side effects, compliance, and labs, making recommendations to clinic provider when appropriate.
  • Educates on administration instructions, potential adverse side effects, warnings, etc.
  • Conducts benefits investigations, assist with patient assistance program evaluations and documents Specialty Pharmacy prior authorizations.
  • Completes enrollment and on-going adherence follow-up for patients referred by Pharmacy Liaison for Specialty Care Management.
  • Completes comprehensive medication reconciliation with EMR.
  • Provides pharmacy support for all enrolled specialty clinics when needed.
  • Supervises Liaisons, including customer service representatives, following applicable federal, state, and other laws, policies and regulations.
  • In all activities, complies with accreditation, legal, regulatory, and safety requirements, including DEMPOS.
  • Provides training, orientation, reassigns Liaisons as needed for optimal operations.
  • Offers direction and development to Liaisons by coaching, identifying training gaps, assigning progressively challenging tasks, and assisting in career development.
  • Contacts physicians and benefits providers as necessary.
  • Completes third-party insurance forms and other paperwork to assist pharmacy staff in processing claims.
  • Maintains a professional and confidential environment for customers at all times.
  • Provides in-service education to pharmacy staff, students, nurses, physicians, and other healthcare practitioners, as needed.
  • Attends and participates in department staff development conferences or other activities related to staff development.
  • Develops competencies as related to position. Maintains own professional and specialized competency through continued education, including attendance of at least one regional or national meeting per year.
  • Meets hospital-wide standards in all applicable areas.
  • Conforms to hospital standards of performance and conduct, including those pertaining to patient rights, to provide the best possible customer service and patient care.
EDUCATION:
  • Requires a PharmD Degree in Pharmacy from an accredited college of pharmacy.
  • Advanced training (e.g. PGY1, PGY2, related Fellowship or BPS certification) is preferred.
CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED :
  • Must be currently registered, in good standing, as a Pharmacist with the Board of Pharmacy in the state of New York.
EXPERIENCE:
  • Requires 2 years of related Pharmacy experience.
KNOWLEDGE AND SKILLS:
  • Requires a comprehensive understanding of the procedures, systems, and equipment used by the Specialty Pharmacy team as well as a high level of proficiency in performing duties and responsibilities.
  • A high level of analytical ability, organizational and personal management skills, and focus is required to ensure the concerted approach to operations.
  • Excellent interpersonal and telephone communication skills; professional in conduct and demeanor.
  • Proficient with standard software used in healthcare office setting (i.e. MS Word, Excel, web browser, etc.) as well as revising and/or implementing information systems.
Additional experience/knowledge preferred:
  • Knowledge and understanding of the federal funded 340B program is highly preferred.
  • EHR/EMR systems, Immunization, and Specialty Pharmacy experience are preferred.
