We are looking for an L.V.N that is kind as well as detail-oriented to work in our Assisted Living community. Must have excellent communication skills and work well as a team. We appreciate self-sufficient, detail oriented candidates with strong communication skills, have the ability to multitask in a fast paced environment while providing compassionate, resident centered care. Supervisory skills required.
To perform this position effectively the candidate is required to have a current California LVN License on file. Experience in long-term care, community health, geriatric and/or rehabilitative nursing is desirable. Knowledge of current State and Federal regulations.
Duties will include:
Administer medications and treatments as prescribed by physicians
Monitor residents' health and vital signs
Assist in the development and implementation of care plans
Collaborate with the interdisciplinary team to ensure residents' needs are met
Provide wound care and other nursing interventions as needed
Maintain accurate and up-to-date medical records
Support residents and their families by providing education and emotional support
Participate in staff meetings and training sessions
Oversee the day-to-day operations of resident care services
Coordinate and schedule resident care assessments and reassessments
Develop individualized care plans in collaboration with residents, families, and healthcare professionals
Ensure compliance with state regulations and company policies
Supervise and support care staff to ensure high quality care delivery
Conduct regular rounds to assess residents' needs and monitor care quality
Act as a liaison between residents, families, and healthcare providers
Address and resolve any issues or concerns related to resident care
Participate in quality improvement initiatives to enhance resident satisfaction and outcomes
