SUMMARY

The Intensive Case Manager (ICM) will provide comprehensive, individualized, empowering, strength-based, and outcome focused services to caseload students and their families. The Intensive Case Manager position serves students and families who are experiencing a crisis. The goal of this position is to help students and families address their most serious levels of concern and to assist them in establishing stability within their situation. The Intensive Case Manager will help students and families gain access to needed resources and the highest level of care (i.e. behavioral, social, physical and emotional) to meet their needs.

The ideal candidate will have experience with conducting assessments and home visits, implementing evidence based interventions, utilizing motivational interviewing, coordinating crisis assistance and collaborating with other youth/human service agencies in the community. The Intensive Case Manager will be responsible for the overall planning and management of implementing the Communities In Schools' (CIS) model of integrated student support.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Develop comprehensive case plans for each student/family receiving services; implement school-based site plans, maintaining required documentation.
  • Collaborate with school administrators, staff and teachers in the implementation of the Communities In Schools (CIS) student services model in compliance with Accreditation standards. The site team may include other CIS employees, volunteers, schools staff or faculty and/or other community partners. This process will include the building and nurturing of school staff relations, relationships to school and community and the engagement of volunteer for the delivery of services.
  • Serve as a member of the school support team that develops the annual school needs assessment. This process will be based on such sources as data collected by school districts and schools as part of overall school improvement initiatives, surveys and discussions with staff, parents and students, and evaluation results from the previous year.
  • Collaborate with the site team to develop and implementation of the school support plan. The plan will include measurable objectives, as well as procedures for delivering school-wide services, targeted and sustained services, monitoring and adjusting services, and evaluating and reporting effectiveness.
  • Lead and coordinate the delivery of evidence-based services based on best practice and risk factor research.
  • Case manage students through targeted and sustained services that will be coordinated and provided at the school site for identified students.
  • Monitor and adjust services as needed to maximize effectiveness and impact.
  • Implement a systematic data collection plan to evaluate the effectiveness of achieving school and student individualized goals and addressing the needs of individual students.
  • Prepare annual end-of-year report to share with the school and affiliate leadership which are instrumental in planning site services for the following year, as well as contributing to affiliate level reports for partners, the state office and the CIS National Office.
  • Lead and coordinate the delivery of evidence-based services based on best practice and risk factor research. Services provided should fall within the framework of Academic, Behavior Coursework and Parent Engagement (ABC P).
  • Complete appropriate documents relative to capturing services provided to students within seven days of service delivery. Ensure accurate and timely data is entered in CIS Student Database System (CISDM).
  • Work with students’ parents providing parent support seminars (i.e. Triple P Parenting) giving parents simple and practical strategies. Support students and families in crisis and help them navigate systems.
  • Act as a liaison between the students, family, community and school site; connect resources to students and families.
  • Execute weekly one-on-one check-ins with students.
  • Facilitate communication and to support students to problem solve, set goals, and establish patterns to achieve goals.
  • Contact parents by phone, email and home visits to share information and develop plans.
  • Work deeply with family and other natural and community supports.
  • Connect students and families to critical/basic needs.
  • Respond to student/family crisis and/ or trauma.
  • Provide non-clinical counseling.

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.

EDUCATION and/or EXPERIENCE

  • Master’s degree in Social Work, Counseling or a related field required
  • Three (3) years of relevant experience working in education, social work or related position
  • Experience working collaboratively with administrators, staff, parents and/or students
  • Case management experience conducting assessments and home visits preferred
  • Ability to understand and apply basic child and adolescent development principles
  • Must have valid driver’s license

SKILLS REQUIREMENT

  • Demonstrated computer proficiency in Microsoft Office Word, Excel, Outlook, PowerPoint and experience with Google Suite, Zoom and other virtual meeting platforms
  • Excellent written and verbal communication skills
  • Ability to analyze and interpret business periodicals, professional journals, technical manuals, and governmental regulations
  • Ability to write and edit reports, business correspondence, and procedure manuals
  • Ability to effectively present information and respond to questions from managers, clients, and the general public, orally and in writing
  • Strong organizational and time management skills with exceptional attention to detail
  • Culturally competent and experienced working with diverse populations and perspectives
  • Must be self-motivated, well organized, solutions oriented, and able to effectively work independently and within a team framework
  • Strong relationship building skills
  • Ability to work a flexible schedule

PHYSICAL DEMANDS

While performing the duties of this job, the employee is frequently required to sit; use hands to handle, feel and keyboard; reach with hands and arms; and talk or hear. The employee is occasionally required to stand; walk; stoop, kneel, or bend. The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close and distance vision.

WORK ENVIRONMENT

The position is located within the Charlotte Mecklenburg area. Regular and satisfactory attendance and punctuality are required. Regular and satisfactory performance including meeting all process deadlines and stated deliverables is required.
