Chrysalis, a well-established community-based program providing supports and opportunities for individuals with disabilities is seeking an Masters level Mental Health Therapist for part-time employment in our Host Home program. This is an hourly position and will be housed in the Lehi Host Home Chrysalis office.
Job Description:
Provide approximately 12 billable hours of individual therapy to male and female adolescents and adults weekly.
Coordinate with other members of the treatment team including DCFS Case Workers, DSPD Support Coordinators, Behavior Analyst, Area Supervising Therapist, and Residential Manager.
Attend weekly staff meetings and monthly Child and Family Team Meetings (DCFS) and/or annual Person-Centered Service Plan (DSPD) meetings. Follow Medicaid, Medicare rules for billing and service delivery.
Complete mental health assessments, treatment plans, and case progress notes in electronic billing system.
Assist with internal auditing and preparation for audits.
