We are seeking passionate and team-oriented NPs and PAs to join our fast-growing team.

Job Duties

• Manage a patient panel with a broad array of patient needs, conducting a mix of acute, chronic, and well visits.
• Treating patients in-office as well as conducting occasional tele-health visits
• Ongoing collaboration with in-office teammates via daily huddles, as well as with virtual clinical teams
• Utilization of your specific clinical training and opportunities to perform in-office procedures.
• Assist the patient on their care pathway by facilitating referrals to other healthcare professionals and medical facilities.

Requirements

• Completed an accredited NP or PA program with a national certification.
• State licensed in California.
• In the past 5 years, practiced as an Advanced Practitioner for at least:
  • 2 years in an outpatient primary care setting, OR
  • 1 year in an outpatient primary care setting, coupled with either a 1-year primary care fellowship or 1 year in an urgent care setting.

Preferred

• Experience with specialty procedures is a plus.
• Bilingual (Spanish)
• Prior experience in cardiology

Compensation: $90,000 - $140,000 per year

This is a full-time role based in our family of Pomona Valley region clinics. Chaparral Medical Group clinics are in Chino, Claremont, Pomona, Rancho Cucamonga, San Bernardino and Upland.
