Central City Concern is Hiring a Mental Health SUDs Clinician Near Portland, OR

The Blackburn Center is an innovative program that offers integrated housing, health care, and employment services with the long-term goal of eliminating homelessness. We seek to change how these services are delivered to all people, especially vulnerable populations.
The Mental Health Clinician works as a member of a multi-disciplinary behavioral health team who provides clinical care within a care team providing close collaboration & coordination with clients enrolled in care at the Blackburn Center. The Mental Health Clinician is responsible for the delivery of care to the population assigned to the behavioral health home, ensuring care that is accountable, comprehensive, proactive, well-coordinated and client centered. The care team works to address all treatment needs, including crisis management, substance abuse treatment, medication management, physical health treatment, mental health treatment, accessing benefits and entitlements, accessing appropriate and affordable housing, providing counseling, case management and integrated dual diagnosis treatment. This position has access to mental health billing and service records.
Central City Concern is looking to hire a Mental Health Clinician for our Blackburn Center!
Schedule: Monday-Friday 8:00am-4:30pm

MINIMUM QUALIFICATIONS:
Must meet qualifications for:
QMHP-R or QMHP
CSWA
LPC-a or LPC
LMFT-A or LMFT
LCSW
Must have a Master's Degree.
CADC certification preferred, or the ability to obtain one within 1 year.
Has the competencies necessary to:
Identify precipitating events
Gather histories of mental, emotional and physical disabilities, alcohol and drug use, past mental health services and criminal justice contacts
Assess family, social and work relationships
Conduct a mental status assessment
Document a multiaxial DSM diagnosis
Write and implement or supervise implementation of a treatment plan
Conduct and document a mental health assessment
Provide mental health treatment and rehabilitative services within ones scope of practice
Must be registered with appropriate Oregon State licensing board to meet QMHP requirements.
Must meet CCC privileging requirements as required by FTCA.
Must have current CPR certification prior to start.
Bi-lingual (English/Spanish) preferred.
Must become a certified Director's Custody designee in Multnomah County within the first 6 months of hire.
If using a vehicle for agency use: Must possess a current driver's license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern's Fleet Safety policy; pass an initial drivers training within 60 days of being an approved driver and continued recertification training; and maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.
Must pass a pre-employment drug screen, TB Test, and background check. This includes clearance by the DHS Background Check Unit.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Work as a member of the Blackburn Center multi-disciplinary team to deliver comprehensive, client centered, and trauma informed integrated services.
Actively participate in the integration of services at the Blackburn Center and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
Proactively identify opportunities for improvement within Blackburn Center workflows and logistics. Participate in review of Blackburn Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
Build understanding of and appreciation for diversity among Blackburn Center clients and support their connection to culturally responsive and specific services as requested.
Collect information regarding clients' history through a combination of interviewing, records review, and consultation, after obtaining written consent.
Conduct and document Mental Health Assessments, including mental status exam, DSM diagnosis, and clinical formulation.
Develop care plan with client with assistance & input from all of the clients care team providers.
Participate in outreach activities necessary to offer services and engage participation in a non coercive manner. Escort clients to meetings and appointments as necessary.
Participate in evaluating clients for risk of harm & initiate Directors Custody transport holds when appropriate.
Participate in clinical staff meetings and case conferences with team members in order to utilize interdisciplinary expertise and facilitate individual treatment plans for clients;
Collaborate with Residential facilities, Adult Foster homes or other housing providers to provide necessary treatment services and coordinate residential and non-residential treatment and service planning.
Participate in admission and discharge planning for any client needing hospitalization or subacute level of care. Client contact must be made within one working day of admission.
Monitor and assure that the health and safety needs of clients are met.
Maintain client records using an electronic medical record. Must be documented as required by program standards and funding requirements including documentation of clinical interactions, behavioral health assessments & individual service plans.
Participate in regular supervision from the Clinical Supervisor, seeking additional supervision whenever necessary. Minimum requirement two hours of documented supervision per month.
Attend all mandatory CCC trainings in a timely manner.
Conduct educational and therapeutic groups for clients, using approved curriculum.
Coordinate services with all members of client's support network.
Participate in quality assurance and quality improvement activities as directed.
Participate in evaluating clients for risk of harm & initiate Directors Custody transport holds when appropriate.
Assist with the development of an integrated treatment program.
Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediate reporting of any PHI breach of CCC or outside provider health records to the CCC Legal and Quality departments, as well as to the program administrator.
Perform other duties as assigned.
