Overview:

Licensed Registered Nurse providing general supervision, coordination, and direction of the medical, nursing, and other services of the agency. Participates in all activities relative to the professional or other services provided. Ensures the overall quality of the staff and services in the Home Health program.

  • Organizes and directs the agency’s day-to day functions.
  • Ensures that an assessment/evaluation is made by a team member, providing a data base to plan and initiate care of the patient.
  • Makes certain there is a plan of treatment established for each patient.
  • Oversees continuing assessment and evaluation is provided in accordance with the patient’s response and progress as related to the course of his or her disease or illness and the plan of treatment.
  • Is available to assist with health care decision making, regarding patient care.
  • Ensures the initial plan of treatment and subsequent changes are approved by signature of the attending physician and carried out according to his direction.
  • Confirms the total plan of treatment is reviewed by the attending physician as often as the severity of the patient’s condition requires and shall be reviewed at least every sixty (60) days.
  • Ensures information is available to the attending physician on an ongoing basis and is timely, accurate, and significant of change in clinical status or condition.
  • Provides information to the Administrator and guidance requested as is necessary to carry out assigned duties; notifies the Administrator or designee of delays or absences within established timelines.
  • Remains available for consultation at all times during operating hours of the agency.
  • Reviews all client referrals for appropriateness.
  • Assures timeliness of billing/reimbursement tasks to ensure fiscal responsibility of the agency, including timeliness of daily OASIS and 485 completion.
  • Responsible for oversight of all agency health care personnel.
  • Participates, and is involved in, employment decisions affecting home health care personnel, including the qualifications of all personnel to meet the needs of clients.
  • Is responsible for evaluations of all direct reports and provides requested input for other positions within the agency.
  • Responsible for orientation and in-service education for all agency health care personnel.
  • Oversight of medical supplies and working with medical records.
  • May make home visits as deemed necessary for quality patient care and for supervisory and training purposes.
  • Attends and completes annual mandatory education requirements and documents the same.
  • Uses safety precautions as appropriate for patients, and self, e.g. proper positioning of self, good body mechanics, universal precautions and defensive driving skills when applicable.
  • Knows and is able to train on the evacuation plan for the office.
  • Maintains knowledge of, and adheres to, all applicable laws, rules, and standards related to the industry.
  • Maintains RN licensure in good standing in the State of agency.
  • Reports, as appropriate, actual or suspicious activity related to fraud and/or operational abuse of laws, regulations and standards.
  • Demonstrates knowledge and competence in the aspects of Personal Care Aide training requirements.
  • Works cooperatively with others; works out issues and problems with others assertively.
  • Follows the line of authority when one-on-one problem solving doesn’t work.
  • Contributes to a positive atmosphere in the office as well as in patients’ homes.
  • Willing to help others when needed.
  • Possesses personal sensitivity to the needs and experiences of others and a non-judgmental attitude towards persons of differing standards, values, lifestyles and ages.

  • Acts within Scope of Practice and Best Practices when involved in directing or providing patient care.
  • Meets Signature Home Health standards for attendance and punctuality; maintains professional manner in dress and in speech.
  • Follows agency policies.
  • Maintains confidentiality of staff’s, patients’ and families’ private health and other information.
  • Attends agency staff meetings as required.
  • Functions as a member of a team of caregivers, promoting team communication and support.
  • Meets productivity expectations for this position; organizes tasks and manages time effectively.
  • Documents client services legibly, accurately and in concert with agency standards.
  • Can work well alone, without supervision.
  • Asks appropriate questions when unsure of an assignment, takes initiative in areas expected of him/her.
  • Uses good judgment in difficult situations.
  • Able to problem-solve effectively when the home environment is not physically safe for staff or the patient.
  • Able to read, write and verbally communicate effectively in English.

 

  • Must possess a nursing degree from an accredited college or university
  • Must possess a current, unencumbered, active license to practice as an RN in this state
  • Minimum 2 years’ experience in home health as an RN
  • Prefer experience with HomeCare HomeBase EMR software and R2 Access experience 
  • Minimum 2 years’ experience in a nurse supervisor capacity in a healthcare setting
  • Knowledge and competence in the aspects of personal Care Aide training
  • Work independently without supervision
  • Use good judgment in difficult situations
  • Problem solve effectively
  • Possess personal sensitivity to the needs and experiences of others and a non-judgmental attitude towards persons of differing standards, values, lifestyles, and ages
