Job Description: Position Summary Provides in-office and community case management services (e.g., phone calls, home visits, housing and financial counseling; referrals/linkages). Provides individual and group therapeutic interventions that facilitate personal growth, skill building, identification and use of natural supports, and use of community resources. Responsible for assisting with assessment and personal growth planning, linking clients to appropriate services, advocating on their behalf, providing support, monitoring effectiveness of services and providing crisis services as needed. Participates in a variety of cross-functional teams and performance improvement initiatives. Qualifications/Requirements: Five (5) years of paid experience in the mental health or related human service field. Bachelor’s degree in a human service related field.
