Description
The Center for Urban Community Services, (CUCS), is an acclaimed human services organization and one of the nation's largest providers of housing and social services. CUCS is a leader in implementing new practices, sharing knowledge and assisting in shaping local, state and national strategies, so that persons who are homeless, low-income, living with mental illness, or have other special needs can live successfully in the community.
Paul's Place Safe Haven invites applications for the Safe Haven Case Manager position at our Paul's Place program which opened in September 2022. Paul's Place is a co-located 24 bed Safe Haven and 24 hour/365 day Drop-In Center.
Safe Haven programs are intended to provide a higher level of care in a more flexible environment than traditional shelters, to currently street homeless people who may have a difficult time engaging in structured services, have longer histories of homelessness and co-occurring mental health and substance use disorders. The ideal candidate will have demonstrated experience with difficult to engage populations.
The Safe Haven Case Manager will provide a full range of person-centered, strengths-based clinical services to a caseload of approximately 8-12 clients. Services include case management, housing placement services, entitlements assistance, assisting clients with ADLs, assisting clients in keeping their units to DHS cleanliness standards, advocacy, referrals and information, crisis intervention, engagement, monitoring, regular outreach, psycho-education, and coordination of care with onsite and offsite psychiatric and medical providers. The primary goal of case management is permanent housing placement. Documentation must be thorough and clinically appropriate demonstrating work toward client established goals.
The Safe Haven program works in close collaboration with the outreach teams from whom the program receives client referrals. This entails working together on housing applications as well as sometimes street outreach to engage clients, and regular case conferencing.
The Case Manager is responsible for working with an assigned group of clients to help them obtain housing, help clients live full and satisfying lives in the community, help their team and program to function well and meet their contractual obligations including achieving contracted housing placement goals, and striving as appropriate to their role, to fulfill the CUCS programs' core purpose, core values and vision statements
Requirements
Experience and Education Required:
  • CM 1: High school diploma and 2 years experience
  • CM 2: Bachelors degree or HS diploma or equivalent and 4 years of relevant work experience
  • CM 3: BSW and 1 year relevant experience (excluding fieldwork) or Bachelors degree and 2 years relevant experience or HS diploma and 6 years relevant experience
  • CM 4: 3 years as CM 3 (internal only)
  • CM 5: 3 years as CM 4 (internal only)
  • For applicants without college degrees, every 30 credits can be substituted for 1 year of experience
  • Demonstrated ability to provide services to a specialized population, computer literacy and good written and verbal communication skills are required.
  • Must be able to work effectively as part of a multidisciplinary team
