Case Manager/Adult Behavioral Health Practitioner

WAGE RANGE: $53,000-$62,000/annually

JOB SUMMARY: Catholic Charities is seeking a self-motivated individual who is dedicated, resourceful, compassionate, trauma-informed, and culturally aware to serve as a Case Manager/Adult Mental Health Practitioner at its Dorothy Day Residence.

The Case Manager/Adult Mental Health Practitioner work with residents of Dorothy Day Residence in downtown St. Paul who have been identified as having a barrier in retaining their housing due to mental health. The Case Manager will help the assigned clients learn to use mental health strategies and skills, health care and community resources to better manage symptoms, meet their independent living needs to achieve a higher quality of life and will provide services that are clinically informed, rehabilitative and enable the client to develop and enhance psychosocial stability, personal and emotional adjustment, social competencies, community skills and independent living skills; supporting the client in strengthening skill areas needed according to their treatment plan.

ESSENTIAL FUNCTIONS:

  1. Assessing functional strengths, resources and needs of the residents assigned to caseload.
  2. Developing individualized, person-centered, trauma informed treatment/community support plans
  3. Provides services to clients in Hennepin or Ramsey County.
  4. Responsible for billing 30 hours per week.
  5. Implementing the interventions identified in the plan.
  6. Ensure confidentiality of all records and interactions
  7. Coordinate services with Catholic Charities Integrated Services, county case manager, family/guardian of client, work personnel, medical personnel, etc.
  8. Develop therapeutic relationship with team and client.
  9. Attend supervision meetings and participate in team-based decisions regarding all clients and services and participate in clinical supervision as required by the grant.
  10. Maintain client files, update client files, update treatment plans a minimum of once every six months and do all required documentation as specified in DHS and/or grant guidelines.
  11. Responsible for reviewing and ensuring the relevance, accuracy, and timeliness of data and reporting for purposes of accountability and client outcomes.
  12. Other duties as assigned.

MINIMUM QUALIFICATIONS/EXPERTISE:

  • Has a current MN Social Work License (LMFT, LPCC, LPC, LSW, LGSW, LISW, LICSW) or is able to attain one for employment.
  • One year of experience as a case manager/care coordinator with Adult Clients with high needs
  • Experience in individual, family, and group counseling preferred.
  • Without licensure, three (3) years of experience in Case Management with Adult Clients with high needs.
  • CPR/First Aid Certification and Narcan training required or obtained within three (3) months of hire.
  • Has the experience and/or ability to work with diverse and/or low-income populations, including those who may be experiencing homelessness, unstable housing, and/or mental illness.
