Community Horizons LLC is Hiring a Personal Care Assistant Near Plover, WI
A local man is looking for assistance with personal care tasks in his home during weekdays only! Community Horizons is assisting with recruitment efforts for this individual through the self-directed supports program. If you’re dependable, reliable, and want to help someone who will make you feel at home, please consider applying! This is a great opportunity if you’re looking for part time employment for some extra spending money.
Hours available: What works for you? Our member needs help mornings, afternoons, and evenings. Pick a shift or pick them all! No overnight shifts. d
- Saturday and Sunday, 8:00 - 9:30am and 12:00pm - 1:00pm
- Friday, Saturday, and Sunday evenings, 7:00 - 8:00pm
Wage:
- $20/hour (double time on holidays!)
Tasks may include, but are not limited to:
- Assist with dressing/undressing and changing clothes if needed
- Hands on support with showering/bathing on Monday/Wednesday/Friday shifts
- Help with brushing teeth, shaving, applying deodorant, trimming nails, and emptying a catheter bag
- Assist with manually feeding meals
- Some light cleaning may be required
- Assist with transfers utilizing a hoyer lift (will train on this equipment if applicants are not familiar)
Who we’re looking for:
- Someone who is reliable, dependable, and friendly
- Comfortable with hands on personal care supports (CNA preferred but not required; willing to train!)
- An acceptable background check
- Great communication skills and ability to take direction

Community Horizons offers support for individuals enrolled in self-directed programs through Managed Care Organizations or IRIS (Include, Respect, I Self-Direct) throughout Wisconsin. Self-direction gives program participants the choice over who provides services and how they receive them. Community Horizons is not the employer on record; members make all hiring decisions with the involvement of a third-party fiscal agent responsible for payroll.
