Must be willing to travel/cover Region B: Okaloosa County (Crestview, Niceville, Fort Walton Beach, Destin) and Walton County (Freeport, Defuniak Springs, Santa Rosa Beach)
*Currently do not have a high census count in this area, position will start PRN and grow into full time position as census increases* Will help to provide some staffing assistance in Region A (Milton and other areas close to Region B)
while Region B expands.
Qualifications:

• Physical Therapy License active in state of Florida
• Able to pass AHCA Level II Background Screening
• Current CPR certification
• Must have a minimum of 1 year work experience in Home Health to include Medicare/OASIS experience.
• Must be able to commit to PRN Minimum productivity requirements to start with expectation to grow into FT position within first 90 days of hire. Must be able to provide at least 2 dedicated full working days each week while under PRN status.
