Care Resource is Hiring a COMMUNITY HEALTH WORKER EHE Near Miami, FL

ESSENTIAL JOB RESPONSIBILITIES
Attend and participate in outreach events and activities in the field.Some non-standard work hours may be required.
Identify and work with high-risk populations in collaboration with local stakeholders.
Provide information to the community on issues of population health significance.
Provide navigation and linkage to access medical care
Conduct or coordinate volunteer training and management to assist with health outreach and client/community education efforts
Conduct outreach visits to clients
Work with identified community partner(s) to establish rapport and assist clients with linkage to care among those who have been lost to follow-up or who have never accessed care.
Prepare and present education/ cultural competency sessions on specialized topics
Network & collaborate with other community organizations for LGBTQ wellness, women's health, and the empowerment of vulnerable populations
Responsible for gathering and sharing information about issues pertaining to that specific community and building strong professional relationships with key community members
Develop and maintain an up-to-date database of community contacts within the community, including individuals, groups, and other sources beneficial in outreach initiatives into the community
Help clients to access services by assisting in scheduling appointments, reminding clients of appointments, accompanying clients to appointments when necessary, and follow-up on missed appointments
Under the direction/supervision of the coordinator/manager, the CHW is the key between the patient and the required assistance or services they need
Communicates with patients, medical staff, and primary care provider if necessary, to provide appropriate, timely, and cost-effective care support.
Enters and maintains all demographic information, maintains all assigned databases and logs to ensure data is complete and accurate
Demonstrates positive and effective interpersonal skills when dealing with patients, visitors, peers, and other health care team members
The liaison will provide real-time solutions to assist with paperwork and filings, assist with telehealth medical visits, and bypass the client's technology shortfall by offering our internet connection or devices, to be used in the field
Safety
Maintains agency guidelines relating to safety, outreach and confidentiality.
Ensures proper hand washing according to Joint Commission and Centers for Disease Control and Prevention guidelines.
Understands and appropriately acts upon assigned role in Emergency Code System.
Understands and performs assigned role in Health Center Continuity of Operations Plan (COOP).

JOB SPECIFICATIONS
Education:
High School diploma required.
Training and Experience:
Certified Community Health Worker (CCHW) certification or equivalent experience preferred.
Completion of HIV-501 training within 6 months of hire date.
Sensitivity and empathy in working with marginalized communities.
Knowledge base of clinical and preventive health standards of care, promoting wellness, and of acute and chronic diseases/conditions within the primary care setting.
Performance involves weighing and evaluating factors requiring judgment, written / analytical ability, and problem solving.
Job Knowledge and Skills:
Bilingual (English Spanish) is preferred. Computer knowledge should include Microsoft programs and familiarity with electronic health records. Excellent organizational, communication, problem-solving and teamwork skills are required. Ability to work independently and handle stress appropriately. Ability to work cooperatively with medical, pharmacy and other supporting staff. Ability to work with multicultural and diverse population is required.
Contact Responsibility:
The responsibility for internal and external contacts is frequent and important.
Own transportation is required.
Physical Requirements
This work requires the following physical activities: constant talking in person, talking on the phone, hearing/visual acuity and exposure to blood borne pathogens and hazardous materials. Frequent, walking, sitting and bending. Occasional standing, driving and climbing may be required. Work is performed in office settings.
