ABOUT THE JOB
MU Health Care is looking for an enthusiastic and knowledgeable Ambulatory Clinical Pharmacist to fill a PRN position and be a key player in our healthcare team. Working under collaborative practice, the ambulatory care pharmacist will be responsible for providing centralized anticoagulation (warfarin) management, disease state management within a family medicine clinic, and supporting population health quality initiatives by completing medication reviews and collaborating with providers to optimize medication therapy, improve medication adherence, promote wellness, prevent disease, and improve outcomes. You will play a crucial role in educating patients and serving as a valuable resource for prescribers. We value your pharmacist license in Missouri, and previous experience or completion of an ASHP accredited community/ambulatory care residency would be a great asset.
