BronxCare Health System is Hiring a Coordinator Infant/Toddler Program Near Bronx, NY

Overview:

Serve as the Healthy Families services Coordinator for the Maternal Infant and Early Childhood Home Visiting Services and for all children and families enrolled in the program. Coordinates staff in the implementation of the MIECHV benchmarks/state performance targets as required by the gran.
Assure that all Healthy Families America/New York best practice standards are met. Will make home visits to ensure fidelity of the home visiting model.
Ensure that all activities are in adherence with all state requirements and grant regulations.

Responsibilities:

1. Describe responsibility:

- Establish and maintain positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of BronxCare Health System and its employee.

2. Describe responsibility:

- Contributes to and participates in the Performance/Quality Improvement activities of the assigned department. Contribution and participation includes data collection, analysis, implementation of and compliance with risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, consistent adherence to the specific rules and regulations of the BronxCare Health System (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and (d) Patient and Customer Service, (e) Employee adherence to the Organizations Policy, Procedures and Regulations.

3. Describe responsibility:

- Responsible for coordinating staff in the implementation of the MIECHV benchmarks/state performance targets as required by the grant. Will make home visits to ensure fidelity of the home visiting model.
Meet with staff once a week to ensure that all responsibilities are being met.

4. Describe responsibility:

- Coordinate staff in developing and implementing all Parent-child interaction activities with the use of the evidence based curriculum and will be responsible for tracking all home forms that are required to be collected and entered into the healthy families MIS system.

5. Describe responsibility:

- Coordinate and ensure that all children (0-3) are monitored in their development. Ensure that staff is conducting developmental screens, and referrals are done within Healthy Families guidelines.
Ensure that all behavior concerns related to the children are addressed and activities for early language are implemented as per Healthy Families guidelines.

6. Describe responsibility:

- Coordinate staff in connecting participants with a primary care provider and assisting with continuity of insurance. Coordinate staff in providing information and programming related to health, tobacco cessation, and other substance abuse needs.

7. Describe responsibility:

- Coordinate staff in conducting all depression screens in accordance with Healthy Families guidelines as well as make any and all necessary referrals.
Coordinate staff in conducting all intimate partner violence screens and referrals in accordance to the Healthy Families guidelines.

8. Describe responsibility:

- Coordinate all programming related to Continuous quality improvement related to the MIECHV/Healthy Families grant such as the breastfeeding target which includes psycho-education for participants and support groups.

9. Describe responsibility:

- Coordinate staff in providing information and programming related to safe sleep, pre term birth, post partum care, and well child visits in accordance of the Healthy Families guidelines and the evidence based curriculum used by the program. Will assist in the coordination and implementation of staff training with Administrative staff.

10. Describe responsibility:

- Ensure that all staff educate families about child injury, child maltreatment, and implement activities developed to address all 10 risk areas on the parent survey that assess for risks and adverse experiences of the parents.

11. Describe responsibility:

- Will make home visits to oversee and assess staff and families as needed.

12. Describe responsibility:

- Assist with special projects and perform other duties as delegated by the Administrative staff.

13. Describe responsibility:

- Responsible for establishing and maintaining relationships with community based services, such as daycare facilities, preschool programs and Head start programs to assist in discharging children
(3-5) as they age out.

14. Describe responsibility:

- Work with the Program Administrator to develop specific plans for the implementation of program activities and all date entry.
-Responsible for overseeing timely data entry and keeping participant files up to date.
-Attend all program related meetings and represent the program administrator when she is not available to attend state /regional meetings.

Qualifications:

- Prior Social Work or Education experience working with infants and toddlers.

- Prior experience in supervising staff preferred.

- 3 or more years of experience working with children and families in a home visiting program.

- Bachelors degree in social work/ childhood education or other human services related field with at least 2 years post experience is required.

- Masters degree/ or current Masters program enrollment preferred in an accredited school for early childhood education/social work, or other related field such as human services or psychology.

- Bachelors Degree in Social Work or Allied Field

- Computer Literacy

- Skilled in identifying problems and brainstorming potential solutions
Good at planning and organizing

- Empathetic and sociable. Highly diplomatic.
Excellent at written and oral communication, and presentation

- Passionate about connecting with parents and children.
