Brockton Hospital is Hiring a RN Case Manager Near Brockton, MA
Provides case management to managed care members across the entire spectrum of health care (i.e. acute care, homecare, skilled nursing and rehabilitation). Performs on site/telephonic pre-admission, concurrent and retrospective review of all inpatient admissions. Collaborates with the primary care physicians, member and family to develop a case management plan that addresses the members needs, including psych-social, medical and service issues, utilizing the nursing process of planning, assessment, coordination, implementation and evaluation to achieve maximum case management outcome. RN with current Mass License. Previous experience in Utilization Management/Quality Assurance/Case Management. Recent Medical Surgical experience in a hospital setting required. Proficient in MS Word, IDX and Hospital Computer systems. Excellent verbal and written communication skills.
