Job Description

JOB DESCRIPTION:

  • Care Coordinators are social services individuals who provide comprehensive, local support to Service Members and Families in the context of a supportive professional relationship. The Government does not have a requirement for Care Coordinators to be Licensed Clinical Social Workers (LICSW). Care Coordinators meet with Service Members and Families in their homes and in other local settings at times that are convenient for the Service Member and/or family. Care Coordinators are trained in military culture, coaching and counseling and service coordination and meet the continuing education and/or licensing requirements to maintain their credentials.
  • Care Coordinators have access to clinical supervision and peer support through regular team meetings and constant inter-communication and collaboration with each other and with other professionals from military, VA, and local civilian programs and practices.
  • The contractor shall provide all services listed to qualified members within three business days of an intake. The intake call or referral will determine the type of care required. All intakes will be considered clients after initial call.

JOB REQUIREMENTS:

EXPERIENCE:

  • Highly skilled clinician/case managers, Care Coordinators do not have to be licensed clinical social workers (LICSWs) or other licensed or license-eligible mental health professionals, who possess a minimum of a master’s degree in mental health or a “related field”.
  • Must speak English fluently.
  • Must have a current and valid driver’s license.

EDUCATION:

  • A bachelor’s degree in social work, Counseling, or another Human Service-related field. (Desired, but not required)
  • Continuing Education: Care Coordinators shall complete 12 hours per year of relevant clinical training (e.g., PTSD training, MBTI training).
