Responsible for the coordination of all activities related to resident care conferences with residents, family members, and members of the interdisciplinary team. Completes MDS sheets and ensures facility compliance with reporting. Coordinates the quality improvement programs within the facility serving as a resource to department heads in the development and maintenance of department specific programs
Essential Job Responsibilities:
Coordinates all activities related to resident care conferences.
Tracks, assigns, and coordinates resident care conferences.
Ensures Resident Care Planning is coordinated.
Ensures timely completion of MDS and submission.
Coordinates quality improvement program for facility.
Coordinates all activities related to facility program.
Serves as a resource to department heads in the development of department based programs
Coordinates and chairs quarterly meetings.
Tracks and reports progress of interdepartmental activity.
Assists in the development and implementation of policies and procedure relating to QI. QI.
Assists department heads in addressing issues raised through quality indicators and their timely resolution.
Demonstrate initiative in self-development.
Attends all relative meetings including department head, supervisor meetings and other meetings mandated by Director of Nursing Services.
Evaluates own strengths and weaknesses and seeks out resources to help develop in needed areas.
Attends all mandatory education in-service programs so as to be up-to-date with annual mandated programs.
Demonstrates currency in issue, regulations, and laws pertaining to long term care industry. Special projects and other duties as assigned are completed.
Through policy development, enforcement, review, ensures that all Local, State, and Federal rules and regulations are adhered to.
Observes all fire and safety related rules and regulations.
Takes any steps required to ensure a safe and secure environment for residents, families and staff. Performs additional duties as assigned. Exhibits teamwork mindset in the daily activities of the job. Promotes a sense of unity and teamwork in own work area and throughout the facility.
Is responsible team member who is honest and accountable for own actions.
Supports the member of their team.
Acts as a role model by promoting cooperation between departments
Qualifications
Educational / Minimum Requirements:
Graduate from an accredited School of Nursing.
MDS experience is preferred.
State/Federal Mandated Licensure or Certification Requirements:
Current Connecticut State Registration.
