Job Description
We are seeking a Caregiver/Home Health Aide to become a part of our dynamic team! You will provide ADL assistance to consumers in the consumer's home.
Responsibilities:
- Provide routine individualized care to the elderly, convalescents, or persons with disabilities
- Monitor and report changes in consumer health status
- Provide personal care, such as bathing and dressing
- Provide individualized care following a modified care plan
- Medication maintenance
- Teach family members ways to assist consumers
Qualifications:
- Experience in hospice, home health, or other related fields
- Positive and patient demeanor
- Ability to build rapport with consumers
- Ability to handle physical workload
- Excellent written and verbal communication skills
