Breaking Ground operates nearly 4,000 units of housing across New York City, along with housing in upstate New York and Connecticut. Supportive housing - affordable housing paired with services designed to help people maintain their homes for the long-term - is widely recognized as a proven and cost-effective solution to chronic homelessness.

But we didn't stop at providing housing. Breaking Ground's programs and services help people experiencing street homelessness - especially those who have been on the streets the longest - to come indoors. Through our Street to Home outreach program and transitional housing resources, we help people get, and stay, on the path to a permanent home.

Reporting to the Assistant Program Director, Healthcare Initiatives, the Harm Reduction Coordinator, Outreach develops and implements Harm Reduction programming across the Street Outreach portfolio. The Harm Reduction Coordinator provides clinical oversight of three outreach-based Harm Reduction Specialists and partners with site-based harm reduction staff, including Harm Reduction Coordinator for Transitional/Permanent Housing, on implementing evidence-informed, person-centered harm reduction programming across the outreach portfolio. This includes hospital coordination and follow-up for substance-use related hospitalizations and emergency room visits, in addition to best practices in use assessment and safety planning.

This position's schedule will be primarily Monday - Friday, 9:00am - 5:00pm with some evenings and weekends as needed to provide programming support. The position will be expected to have a minimum of 3 days on site with up to 2 days remote work as programming permits. In-person days will likely include field-based outreach which may involve extended periods of work outside, including extended periods of walking and service provision in varying weather conditions.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Lead the implementation of standardized, scalable, person-centered harm reduction practices across the outreach portfolio, drawing on an existing Breaking Ground Harm Reduction Toolkit

Provide clinical oversight of a group of three outreach-based Harm Reduction Specialists

Collaborate with Harm Reduction Coordinator for Transitional/Permanent Housing and Assistant Director of Healthcare Initiatives to develop and deliver training on harm r eduction practices and equip Harm Reduction Specialists with tools to do the same

Partner with various staff members to support training and education around needlestick prevention and sharps container distribution and maintenance

Partner with Program Development and transitional housing-based harm reduction teams to develop and foster relationships with harm reduction resources and programs and support outreach client connection to same

Liaise with program-based and administrative medical providers to promote access to medications for addiction treatment (MAT), syringe access, and other harm reduction modalities

Partner with harm reduction team, program staff, and Breaking Ground data services team to develop data collection and reporting tools to track harm reduction services, outcomes, referrals, and substance use-related health care utilization

Perform other duties as assigned

MINIMUM QUALIFICATIONS:

Bachelors degree or equivalent experience required ; NYS Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Master of Public Health (MPH), Master of Public Administration (MPA), or similar strongly preferred

Minimum 2 years of experience; working with the homeless, formerly homeless, and/or clinically complex clients preferred

In-depth knowledge of harm reduction practices, including working knowledge of evidence base for harm reduction practices and commitment to remaining up to date on existing and developing best practice models/concepts

Proficiency in Motivational Interviewing practices

Excellent computer skills, including proficiency in Office 365 and electronic case management or health record systems (AWARDS experience preferred)

Bilingual Spanish/English is preferred
