Social Worker (Licensed) position on Staten Island!
Full-time position, 35 hours/week!
POSITION OVERVIEW: TheCCBHCis comprisedofseveralcommunity basedprograms providing direct care toover 2000 clientsannually, both on site and in the community.The Bridgesto Care Social Worker reengages clients who have stopped attending treatment and are deemeda risk toharmself orothers or an elevated risk for hospitalization. Working primarily in the field, the social worker provides in-home crisis stabilization/treatment services/supports and 24/7 emergency crisis response.

KEYESSENTIAL FUNCTIONS: Both in theCCBHCsettingandinthe community, the Bridges to Care Crisis Social Worker:

• Conductscrisisevaluationstoascertainlevelofriskandusesevidencebasedinterventionstomanagerisk
• Usesmotivationalenhancementapproachestoincreaseclient(re)engagementincareandservices
• Assists clients withschedulingappointmentsfor behavioralhealth, andphysical healthservicesandaddress barriers that may impact clients' ability to keep appointments, advocating for clients who need support
• Participates inhospitaldischargeplanning; communicates withservice providers andfamilymembersto increase likelihood the client will successfully transition back to the community and engage in services
• Maintainaccurateandup-to-datecaserecords,e.g.progressnotes,assessments,statistics,andother documentation as required by company and other governmental licensing and funding entities.
• Attendprovidermeetingstopromotecollaboration andcoordinationofservices
• Participateinsupervisionandtrainingactivities

EDUCATIONAL/TRAINING REQUIRED:

• Master'sleveldegreeinhuman servicesfield with appropriate NYS licensure (e.g. LMSW,LMHC, LCSW)
• Experienceworkingwithclientswithseverementalillnessand/orsubstanceuse;
• Experienceworkingaspartofaninterdisciplinaryteam;
• Experienceprovidingcaremanagementorcarecoordinationinamedicalorbehavioralhealthenvironment;
• PriorexperiencewithelectronichealthrecordsandMicrosoft Office

WORKENVIRONMENT/PHYSICALEFFORT:

• Theposition entailsprovidingserviceseitherintheoffice,inthecommunity,as indicated(i.e.,clients'homes hospital),or via telehealth
• Whiletheofficesofthecompanyareaccessiblein accordancewiththeADA,thesitesto whichstaffmayneed totravelmayormaynotbe
• Toperformtheessential functions ofthisjob the candidatemust be able to travel within New York Citycarrying equipment (notebook, forms, laptop, mobile hotspot, cell phone) weighingup to approximately 10 pounds
• Toperform the essential functions of thisjob,the candidate is routinely required to sit(60% of time) and stand (20% of time), and travel toand from appointments usingpublic and private transportation {20% of time) .

Salary : $60,000 per year plus benefits. Clinical supervision towards LCSW may be available.
