The Philadelphia Housing Authority is Hiring a Social Services Coordinator Near Philadelphia, PA

Marketing Statement

Under general supervision of the Social Services Manager, the Social Services Coordinator provides counseling, referral and case management services to residents of PHA, PAPMC and HCV sites. Partners with outside agencies and provides information on available resources and education on use and benefits of these services. Monitors services provided to ensure highest quality of service available for PHA residents and families; performs other related duties.

Salary: $46,500-$48,796.80

Qualifications

Minimum education

Bachelor’s Degree in Social Services, Psychology, Family Services or a related field, AND one (1) or more years in social service program or case management experience; OR an equivalent combination of education and experience.

1-3 years of related experience

Responsibilities

Essential functions

  • Works creatively to address the needs of the community and utilize the expertise of community partners and agencies to assist in life enrichment, supportive services and education.
  • Establishes proactive and positive communication with Manager, residents, neighbors and the community at large through educational programs directed at building self-esteem, positive parenting styles, home management and economic opportunities; communicates effectively with people of different social, economic and ethnic backgrounds
  • Maintains individual resident files including intake information, assessments, services offered, quarterly review and follow up and documentation of contact with residents, providers and families
  • Uses case management system to track case management data, trends and benchmarks
  • Complete assessments of residents and family members and prepared goal action plans that include educational, social and employment strategies as needed
  • Maintains absolute confidentiality of work related issues and PHA information
  • Attends program related or community meetings
  • Provides training and information to residents, property management, and other staff around social services and areas of need
  • Completes needs assessment and determines appropriate referrals to meet the needs and goals of residents;
  • Works with regional partners and forms new partnerships in assigned area within the community to provide resources, services and information to residents;
  • Conducts site visits to monitor and ensure senior PHA residents receive needed social services;
  • Facilitates and coordinates engagement events, virtually or in-person as appropriate;
  • Provides residents with tools to prevent eviction, including financial coaching, repayment planning, and money management skills training;
  • ·Submits monthly reports on resident interactions, services, and outcomes;
  • Refers residents to financial literacy, employment, and training opportunities at PHA’s Workforce Center;
  • Conducts or facilitates workshops on-site at assigned site(s);
  • Works closely with other RPP departments to offer workshops on-site at development including Workforce Development, Senior programming, and Youth programming;
  • Promotes PHA programming and initiatives; and
  • Provides referral services and follow up on areas in need, including: education, eviction prevention, employment, health and wellness, elderly services, reentry services, and financial education and coaching.
  • Other duties as assigned
