The Program Clinician provides individualized coordination of clinical treatment, individual, group, parent/caregiver peer support, therapeutic support, and young adult peer mentoring services. Broker informal supports and establish broad-based community involvement to benefit youth and families of FSS. Work closely with FSS team on development of service plans and action plans for youth and families served. Manage crisis situations with youth and families and serve as on-call clinician. (Families that are involved with the Department of Mental Health who are referred to the F;exible Support(FSS) Program.

Flexible Support Services is a program to assist children and youth diagnosed with serious emotional disturbance (SED) and their families. Services are provided within the scope of the wraparound model, which includes family-driven services, formal and natural supports, and professionals in a team approach.

Responsibilities :

  1. Meet and engage with new FSS referrals in a timely and family-friendly manner.
  2. Engage in a comprehensive and on-going assessment and screening process to determine youth/family functioning across all life domains and including youth and family strengths, needs, and culture discovery.
  3. Conduct specialized assessments (e.g. CRAFFT, Audit-C, CANS, etc.) to address the unique needs of individuals with co-occurring substance use or other co-occurring complex conditions.
  4. Co-create a Community Service Plan with the youth/TAY/YA and family and update as needed.
  5. Develop treatment approaches that reflect and target the needs identified in the comprehensive assessment.
  6. Research, develop, and implement family therapy interventions that are evidence-based or grounded in promising practices (TF-CBT, DBT, MiiWrap, PBS, FBA, ARISE, A-CRA, Practice Profiles, Family and Multi-Systems Therapy, etc) and utilize techniques that have proven effective for home-based worked with youth/TAY/YA and their families.
  7. Provide modified treatment approaches to respond to the unique capabilities of individuals with co-occurring disorders, complex diagnoses, and cognitive delays.
  8. Provide ongoing safety assessment, planning, and on-call crisis support to include on-call mobile support 24/7 as scheduled by supervisor.
  9. Coordinate youth/TAY/YA services to assure that they are integrated, clinically coherent, and culturally sensitive across all treatment providers and formal supports.
  10. Utilize youth/TAY/YA self-assessment data to monitor and collaboratively determine transition in collaboration with youth/TAY/YA, Parent/Caregiver, Clinician, wrap team, and DMH.
  11. Celebrate with youth/TAY/Parent/Caregiver incremental success throughout service delivery.
  12. Identify and coordinate continuity of care approaches and challenges/barriers across providers, school, state agencies, and other supports.
  13. With proper consent, collaborate (face-to-face meetings, by phone, e-mail, or other technology, observations) with third parties (school staff, pediatricians, team members, or other formal and informal) on behalf of FSS involved youth/TAY/YA and/or with the Parents/Caregivers, or other family members.
  14. Support the Parent/Caregiver and the youth/TAY/YA in developing strategies for self-care, identifying respite care providers (both formal and informal), and accessing respite care.
  15. Attend, coordinate, and facilitate: Utilization Reviews, CSP meetings, Clinical Reviews, Hospital Discharge, and other team meetings (including emergencies) as required and clinically indicated.
  16. Ensure that CSPs and other Action Plans are implemented as planned by the youth/family and their team.
  17. Maintain clinical records for accuracy and compliance with BAMSI’s Quality and Improvement and established DMH standards and protocols.
  18. Work cooperatively with other programs within the CF&CS and Adult Services (when needed) division to ensure a comprehensive and consistent community-based treatment approach.
  19. Maintain collaborative relationships with core providers, community providers, community organizations, DMH, DCF, DYS, and Community Service Agencies.
  20. Attend all required internal and external trainings and meetings.
  21. Ensure timely submission of weekly activity sheets and daily progress notes.
  22. Participate in weekly clinical and administrative supervision and monthly consultation, behavior rehearsals, and field observations.
  23. Support monthly and annual consumer satisfaction surveys for FSS.
  24. Other interventions commensurate with clinician’s credentials, experience, and training.
  25. Perform duties consistent with DMH/BAMSI policies and procedures.
  26. Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk. Perform other related work duties as needed or as assigned by supervisor and/or designee.

Qualifications:

  • Master degree in a human services field, is trained in psychology, social work, or other clinical disciplines, and has at least 1 year of experience working directly with youth/TAY/YA living with Serious Emotional Disturbance (SED) or other mental and emotional challenges required.
  • Licensed or license eligible (LICSW, LMFT, & LMHC) a plus.
  • Experience working with SED youth and families required.
  • Experience in clinical treatment, care coordination, and workforce support/coaching.
  • Understanding and experience working with clinical challenges specific to a mental health, behavioral health, and traumatic stress.
  • Experience working with state systems (DMH, DCF, DDS, DYS, & DOE) and CBHI (ICC, IHT, FS&T, TM, & ABA preferred).
  • Broad understanding of DMH regulations and policies.
  • Knowledge of Wraparound principles, phases, and meeting facilitation a must.
  • Strong administrative, organizational, computer, professional writing, and oral communication skills preferred.
  • Willingness to travel up to 75% of the time, in-state within BAMSI region.
  • Must have valid driver’s license in state of residence and own means of transportation.
  • Must be flexible/ available on weekends and Holidays to accommodate families’ needs.
