As a QIDP , you are providing personal assistance to individuals who need help with daily living. The individuals we serve have profound to mild developmental or intellectual disabilities, and as a QIDP you will work with other dedicated Qualified Intellectual Disability Professional's, as well as Direct Support Professionals who share the very same passion for helping and assisting others. Our wonderful staff assist individuals with daily living and promote growth in all areas of needs. At Avancer Homes LLC we provide in home care to persons served in eighteen beautiful residential homes across Ogle and DeKalb County. We are in a place of rapid growth and are very excited to connect with like-minded individuals who are interested in making positive changes in the lives of others!

Our amazing staff works together as a team to promote a positive experience for everyone we interact with. We believe in the value of an individual, people, and we as an agency, are committed to treating our fellow employees and persons served with kindness and respect.

Some responsibilities of a QIDP may include, but is not limited to:

♥ Develop and implement Individual Implementation Strategies for designated caseload of Individuals in the CILA program.

♥ Ensure data collections and corresponding progress notes for each Individual are completed in a timely manner.

♥ Provide training to DSP on the implementation strategies as defined in the Personal Care Plan.

♥ Review of health concern forms with appropriate staff. Monitor follow-up and inform Individual’s home of action within appropriate time frame.

♥ Ensure Individuals are instructed on activities of daily living, community integration, money management, and medication administration.

♥ Attend all psychiatric appointments and provide information and insight on Individuals status.

♥ Maintain an adequate liaison system with guardians, families and Individuals. Be available, through the use of an agency provided phone, for family and guardian contact, general questions and emergency issues.

♥ Provide information to the Residential Manager for filing in each Individual’s file and binder.

♥ Review daily logs of incidents, activities and occurrences for each Individual residing in the homes for program planning purposes.

♥ Ensure that each Individual provides input in the services that they are provided

♥ Be an active member of the Community Support Team (CST).

♥ Cover DSP shifts in homes as needed.

♥ This is not an all inclusive list

** This position does not require to be on call, and does not require scheduling.

Educational Requirements:

Bachelor’s degree in a Social Service field with a minimum of 1 year experience working with Individuals with Intellectual Disabilities. Certified as a QIDP or the ability to become certified.

Special Requirements :

• No disqualifying criminal history

• Certified Direct Support Personnel (DSP) or the ability to become certified

• At least 21 years of age

• Valid Illinois driver's license and ability to maintain a good driving record.

• Ability to drive company vehicles after being approved

• One year experience working with Individuals with intellectual disabilities

• Ability to lift 50lbs

Intermediate computer skills

• Ability to work independently

• Organized with good interpersonal and communication skills

• Basic math and cash handling skills

Tools & Equipment Used:

- Van, automobile, copier, telephone, fax, and computer

Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus.

Work Environment:

Work is performed primarily in offices and units, vehicles, meeting rooms both in the building and at other agency sites.
