Job Summary: Basic purpose or primary function of the job

Provide social work services to maintain and improve functioning of senior clients and their families in accordance with Senior Services policies, procedures and goals. Coordinate with other appropriate community resources to enhance client/family care.

Oversee the coordination of the Kosher Meals on Wheels (KMOW) Program including taking meal and soup orders, completing paperwork for delivery and billing purposes, and supervising the food ordering, preparation, packaging, and distribution of the KMOW.

Job Responsibilities: Principal responsibilities or job duties.

  • Assist with oversight of the programs and services provided to Holocaust survivors
  • Provide supervision to social workers who provide services to Holocaust survivor population, as needed.
  • Conduct multi-functional psychosocial assessments, including depression and mini-mental screenings baseline to augment existing services or as a guide in determining or changing level of service
  • Provide ongoing insight oriented and/or supportive counseling to clients and families referred to Senior Services
  • Provide ongoing case management to clients to maintain their independence in the community
  • Assess clients' financial situation and make appropriate referrals to ensure entitlements
  • Develop with client and family, wherever possible, a realistic plan, both long-term and short-term, including viable options
  • Prepare accurate and timely records, including clinical and progress notes, and three-month outcome measures
  • Share relevant client care information regularly with supervisor.
  • Participate in conferences and share relevant information or resources
  • Utilize all resources of family and community agencies to assist client and best meet the client's emotional, environmental, physical and financial needs
  • Participate in in-service trainings
  • Participate in the Senior Service staff meetings
  • Interpret and represent Senior Services policies to appropriate individuals and agencies
  • Attain and maintain licensure and/or clinical license
  • Attend continuing education conferences/programs
  • Take client KMOW orders over the telephone and provide new clients with appropriate information about the KMOW Program (e.g. cost; if self pick-up or delivery; inquire if any other needs to refer to colleagues at JFCS).
  • Submit the order timely each week to Betty the Caterer (BTC), overseeing the inventory and reminding clients when they haven’t regularly ordered.
  • Take soup orders from Holocaust Survivors, submitting them to BTC, overseeing the inventory and reminding clients when they haven’t regularly ordered.
  • Process new client intakes in client database by entering demographics and payor source, taking initial payments, and forwarding pertinent information to all involved in the process, including Office Manager and Senior Services Intake staff.
  • Alert Senior Services Director of special client situations, for example, if client expresses inability to pay for meals and needs alternative resources.
  • Take payment of any new callers to the program for the first time and provide this to Office Manager.
  • Maintain supply of extra KMOWs to have on hand for those who forget to order.
  • Be present at the Saltzman House on site on Mondays to receive the meal delivery from BTC (this includes unloading meals from the delivery van- heavy lifting is required), and sort meals for orders along with volunteer/staff assistance where needed, in preparation for volunteer pick-up and delivery on Tuesdays (also on site at the Saltzman House), or as coordinated based on holidays and scheduling needs. Lifting is also required to store all bags packed by client in a walk-in freezer.
  • Coordinate any changes and needs with pack out times and pick up times with Volunteer & Senior Services Staff to ensure volunteer and client communication as to any changes, including with holidays and special deliveries.
  • Make sure that KMOWs are packaged properly; KMOWs should consistently have food labels and kashrut labels on each package, and they should be placed in bags labeled as “1 bag of 1 bag” or “2 bags of 2 bags.”
  • Coordinate distribution of those meals weekly to volunteers for delivery. Troubleshoot where deliveries are unsuccessful, such as receiving meals that are returned due to unsuccessful delivery and being present at the Saltzman House at an alternative time for pick-up, when needed.
  • Alert Volunteer Coordinator and/or Senior Services Director of any problem surrounding volunteer pick up/delivery.
  • Be present and coordinate with special food projects and holidays to receive meals for additional deliveries either in bulk or on extra days.
  • Inform KMOW clients via letter of any changes in the order/delivery schedules due to holidays.
  • Coordinate with BTC to update the meal menu based upon need.
  • Make appropriate arrangements for KMOW orders when out of the office for any period. Specifically, two weeks of orders will be taken in advance of departure and/or back-up staff on-site will be provided to take KMOW orders when the staff is out.
  • Contact JFCS Senior Services Department staff about emergency food needs of any caller and make sure information is communicated to a senior staff person.
  • Follow-up on delinquent accounts.
  • Processing monthly payments from BTC for soups and food

Bachelors in Social Services – Masters Degree preferred

  • MSS from an accredited school of social work
  • Knowledge of all social work methods with particular skill in individual, family and group counseling and management of the elderly
  • Extensive experience working with HR systems, specifically Lawson, and Business Objects and Access
  • Advanced Level PC skills, Microsoft Office – Excel, Word, PowerPoint , and Microsoft Access
  • Ability to perform work that is routine and strong attention to details
  • Knowledge of systems and functionality.
  • Ability to work in cross-functional teams.
  • Excellent project management, customer service, organization, and time management skills
