These initiatives will primarily focus on disease management and prevention with the ultimate aim of creating and sustaining thriving communities. With a strong emphasis on quality and clinical support services, the director will also develop and manage policies, procedures, and practices to ensure consistent and effective MADC program delivery in compliance with regulatory requirements. This position requires a high level of fiscal acuity and skills in developing strong working relationships with a variety of internal and external partners to achieve equitable and sustainable services of the highest quality.The director will lead the department in fulfilling the following goals: decrease avoidable hospital utilization; improve overall health through strategic partnerships, particularly with those addressing social determinants of health, and strengthen integration and linkages by wrapping community health programs and services around defined populations, services of the Network and both hospitals.Supports the Mission of Trinity Health and Holy Cross Health.Minimum Licensure/Certification Required (if applicable):Licensed Certified Social Worker-Clinical (LCSW-C) in the State of Maryland or licensed in area of clinical expertise. CPR and First Aid CertifiedMinimum Knowledge, Skills, and Abilities Required:Master's degree in Social Work, Public Health, Health Care Administration, Nursing or health related field. At least eight (8) years of experience providing health/social services to adult populations. At least five (5) years of experience in management, preferably in a health care or community health setting. Strong organizational skills with experience managing complex projects with a variety of constituencies. Demonstrated ability in establishing and maintaining effective, cooperative working relationships with institutional, governmental and hospital staff/departments. Strategic thinker with business development, finance and marketing skills. Strong commitment to achieving optimal fiscal and clinical performance and achieving and maintaining compliance with Federal, State, County and local regulations and accreditations. Demonstrated ability in effectively and efficiently managing human, material, capital and financial resources. Expertise in developing, organizing, implementing, evaluating and improving health education classes and programs for the target populations. Excellent oral and written communication skills, including the ability to develop and make organized, concise and motivating presentations and the ability to work with Holy Cross Health executives, physicians and external organizations Proficient with Microsoft 365 applications including Word, Excel, and Teams Ability to work flexible hours within the office or out in the community.
