JOB PURPOSE:
The Case Manager under general direction of the Lead Case Manager and Director/Regional Director, provides comprehensive case management to the homeless population by utilizing a trauma-informed care, strengths-based approach in all aspects of therapeutic community living; To provide quality of life and maintain the highest possible ability to function within the community.
ESSENTIAL JOB RESPONSIBILITIES:
Develop effective, trusting relationships with program participants by utilizing a trauma informed care and strength-based approach. Model positive interactions, verbal guidance, and effective communication.
Utilizing the Housing First Model advocate for and facilitate participants access to community resources; housing resources, school programs, vocational opportunities or services, recreational activities and relevant mental health services and benefits; assist participants to develop natural resources and contact social support networks. Assist participants with benefits.
Develop and maintain comprehensive outreach/working relationship with landlord's, property management teams, and community partners on the participants' behalf to develop workable plans to obtain and/or maintain housing.
Evaluate participants' individual needs. Complete timely service plans with participants including short- and long-term goals; provide follow-up and evaluation of goals. Coordinate and monitor services, including comprehensive tracking of participants' activities in relation to service plan.
Document weekly and emergency case notes; submit comprehensive resident files, complete reports as needed; and enter documentation into HMIS system. Prepare necessary incident reports.
Identify and provide emergency crisis services as necessary; make immediate clinical assessments and respond according to accepted crisis intervention methods and techniques; coordinate other services as appropriate.
Maintain on-going communication with multi-disciplinary team across multiple programs to assist participants in obtaining and maintaining permanent housing, services to meet needs, and promote stability.
Provide needed life skills training on a one-on-one or group basis or connection with partnerships. Assist residents in developing a budget and monitor payment of necessary bills. Assist with problem solving, social skills, parenting skills and behavior modification to meet participants' goals of maintaining housing.
Demonstrate exceptional customer service, in everything you do, by placing the child, family Veteran or participant first to support our mission to "Empower people to build better lives for themselves, their families and their communities."
ESSENTIAL QUALIFICATIONS:
EDUCATION: Bachelor's Degree in Social Work, Sociology, Psychology or related field.
EXPERIENCE: 1 years of experience in a social service agency and/or 1 years in a customer service focused environment. Experience with mental health and/or a homeless population is a plus.
ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.
LICENSES : Driver's License with clear record.
VEHICLE : Must have daily use of a vehicle without prior notice.
OTHER: Must be available and willing to travel to various locations and with such frequency as the business need dictates.
