We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers.
The Nurse and Patient Coordination Manager's responsibilities include the oversight of Nurses and Patient Care Coordinators within the organization. The Nurse and Patient Coordination Manager facilitates/coordinates safe, high quality clinical care, treatment, services and workflows for the organization in collaboration with Senior Management, and other clinical managers and support staff.
Requirements
Education: RN with at least two to four years management experience required. BSN preferred. Current New Hampshire RN License required.
Experience: Community health experience preferred. Possess an understanding of compliance requirements and be responsible for reporting on clinical tracking.
