§ Be dependable, reliable and punctual with attendance; plan appropriately for absences when possible.

§ Show initiative and drive within scope of job responsibilities; show initiative without needing direction but ask for clarification, correct errors and request assistance in a timely manner, handle unexpected situations calmly and efficiently to minimize problems.

§ Show commitment to giving quality service and make continuous efforts to improve; consider accuracy & appearance of work, learn from mistakes while taking action to reduce errors and understand the role quality plays in customer satisfaction.

§ Understand and respond to resident/customer needs; give complete and timely responses and always be respectful and courteous while following through with communication efforts.

§ Take responsibility for decisions, actions and results.

§ Communicate effectively with residents, family members, guests, co-workers, supervisors, etc.; actively listen and clearly share information demonstrating effective oral and written communication.

§ Exercise good time management practices while maintaining quality of work; plan and organize work load to ensure job duties are handled efficiently and effectively and provide timely responses to those who request information.

§ Ensure personal hygiene, appearance and dress; present neatly groomed appearance and follow department dress code ensure identification badge is worn where visible.

§ Demonstrate dependability, reliability and punctuality. Report to work when schedule, ready to work. 

§ Other duties as assigned.

Essential Job Duties: 

§ Perform hand hygiene when going in and out of residents rooms. Hand hygiene involves washing hands when soiled or after removing gloves. Alcohol hand gel/foam sanitizer is the other form of hand hygiene. 

§ Assist with transporting residents by pushing the resident who is in a wheelchair or recliner where he/she wants to go or to an activity within the facility, therapy room or beauty shop. Check that resident’s feet are on foot pedals. Use good body mechanics and proper protocol.

§ Answer resident’s call bells within 3-6 minutes to see what he/she needs. 

§ Assist resident with bathing, toileting, and other personal hygiene needs as required.

§ Pass fresh cold water each shift and snacks out to residents per protocol in a sanitary and safe manner. Pay strict attention to resident’s fluid thickness orders or mechanically altered diets.

§ Observe residents for changes and/or problems and report them immediately. Report all changes and/or problems to staff nurse promptly.

§ Maintain resident rooms neat and uncluttered. Straighten closets, bedside stand, dressers, etc. Always ask resident’s permission to go into her/his belongings as their rooms are their homes. Never move items without permission.

§ Make resident’s beds per standard.

§ Correct safety issues regarding resident-care environment or equipment and complete maintenance or housekeeping requests as appropriate to keep environment free of unnecessary hazards. Correct potential hazards immediately or report them to supervisor. 

§ Feed residents requiring assistance and encourage residents as needed. Provide bib/cover-up if needed. If feeding is required, complete in an unhurried manner and feed in appropriate serving order. Serve trays within 15 minutes of arrival to unit. 

§ Take direction from staff nurse on your assigned unit. 

§ Manage time wisely to ensure resident needs are met. 

§ Encourage residents to attend activities and remind them of scheduled activities for the day.

§ Complete items on “to do” sheets and return list to staff nurse prior to end of each scheduled shift. Advise staff nurse prior to the end of each shift if all assigned tasks will not be completed. 

§ Follow Fire and Emergency Preparedness Plans.

Job Summary:

The primary responsibilities of the Direct Care Assistant involve supporting the needs of Summit Square’s residents, such as passing fresh water and or snacks, answering call bells; responding to resident needs, making beds; gathering clean linen; stocking supplies; assisting residents with ambulatory needs; assisting with simple housekeeping chores and assisting with mealtime. It is important to respect and promote each resident’s rights and always maintain a clean and safe home-like environment.

Qualifications:

· Ability to follow directions and complete assigned tasks.

· Ability to read and write with good communication skills.

· Ability to process information accurately, make appropriate decisions and take appropriate action based upon the information available.

· Must hold a current D.C.A. certification.

· Ability to work with confidential information and maintain its integrity.

· Ability to multi-task while maintaining attention to detail and have good organizational skill.

· Ability to work in stressful environment and deal with medical emergency situations.

· Strong customer service skills; maintain a hospitality approach to engaging with all individuals.
