Alta Mira NM is Hiring a Family Supports Service Coordinator Near Albuquerque, NM

Alta Mira Specialized Family Services is a non-profit organization that has been supporting individuals with developmental or intellectual delays or disabilities, and their families, since 1985. We provide services to all age groups. We support over 800 children annually in our Early Intervention Program and 1500 adults in our Family Support Services. REQUIREMENTS

  • Position requires sitting for prolonged periods of time.
  • Occasionally lifts up to 20 pounds and carries small objects from waist to chest.
  • Must be able to type.
  • Ability to distinguish telephone rings and to hear and speak clearly over the telephone.
  • Regularly needs to bend, stoop and reach.
  • Must be able to speak, read and write English.
  • Vision and hearing sufficient to perform the essential functions of the job.
  • Must be able to drive to and from home visits.
  • Must maintain clean driving record.
  • Ability to get along with co-workers and supervisors.

SKILLS

Accuracy, computer literate and/or ability to learn computer programs, knowledge of community resources and intermediate to exceptional verbal communication skills. Bilingual in Spanish and English preferred. Able to initiate and complete work assignments with minimal supervision, to follow through with multiple tasks, directions and responsibilities. Reasoning: Intermediate to Elevated Level. Ability to fill-in for program director &/or staff at specific meetings and to serve on additional committees within & outside of the agency preferred.

EXPERIENCE

BA/BS in related field preferred, or 2 or more years of related experience. 

Position Specific Essential Job Functions

Performs essential job functions with or without reasonable accommodation at an acceptable level.

  1. Initiate and maintain family and individual relationships to assure Alta Mira Family Support Services (FSS) are delivered.
  2. Partner with families, as the experts on care for their family member with a developmental disability, and assure family’s participation in training of direct care providers.
  3. Gather and communicate family input regarding quality of services.
  4. Implement and monitor Family Support Services for persons receiving services.
  1. Develop nurturing relationships with persons receiving services, in order to facilitate desired outcomes and/or meeting needs.
  2. Take initial referrals, schedule and complete initial intake interviews in order to obtain information for planning service delivery.
  3. Develop individualized goals and objectives (Individualized Family Service Plan / IFSP or Individual Service Plan / ISP) with persons receiving services, consulting with other Family Support Services staff and/or other agencies, as appropriate.
  4. Schedule and complete regular Home Visits and/or scheduled observation visits with persons receiving services in order to evaluate and review goals and objectives, quality of services being received, health and safety needs, etc.
  1. Respond to family’s and/or individual’s needs for changes in program or service plan.
  2. Provide on-going Family Support Service coordination for persons who are receiving services.
  3. Implement Service Coordination policies and procedures as defined by funding and regulatory agencies, and including Alta Mira Specialized Family Services.
  4. Act as a resource for persons who are receiving services and other community providers; including providing information about policies and procedures.
  5. Assist persons who are receiving services in locating and accessing needed services within Alta Mira and/or from other agencies/providers.
  6. Provide and monitor services to assure services are provided in natural environments and/or imbedded in the daily routine of the person who is being served.
  7. Work with families, program participants and community resources to promote independence of the person receiving services.
  8. Develop, plan, lead and/or participate in educational, training and outreach activities, including planning and presentation of lectures and/or workshops, as requested.
  9. Share pertinent information with supervisor, team members and/or co-workers regarding work progress, successes, barriers, concerns and other areas that may have an impact on the agency’s services and/or programs.
  10. Participate in the development and implementation of Family Support Services training/orientation programs.
  1. Work with Family Support Services Program Director and staff, and families / individuals receiving services, in the development of a master training and orientation program for all service components.
  2. Conduct initial Introduction and Basic Orientation program for direct care providers.
  3. Schedule and conduct individual orientation and home visit/study with prospective independent contractors.
  4. Performs face to face home visits monthly
  1. Communicate with direct care providers and assist them in understanding special needs of persons receiving services.
  2. Participate in the overall development and implementation of the FSS program.
  3. Complete and submit accurate documentation and reports, as required and in accordance with program standards and time lines.
  4. Assure all family and individual files are complete, up-to-date and in compliance with funding and/or regulatory standards.
  5. Complete and file notes for each contact made with families, individuals and/or direct care providers.
  6. Assure required data is collected, compiled and reported.
  7. Comply with accreditation and regulatory standards as required.
  8. Crisis/Emergency Intervention.
  1. Available for On-Call duty, as assigned.
  2. Accompany staff and/or persons receiving services to emergency room services, as appropriate.
  1. Make referrals to community resources when persons receiving services are in crisis.
  2. Performs other duties as required or assigned.
