Pathway Hospice, the leading locally owned hospice provider in DFW, is seeking an experienced Hospice RN to assist our patients after hours and on weekends. The ideal candidate will be have at least 2 years of dedicated hospice experience and be committed to providing exceptional care for our patients and their families. While our office is in Richardson, TX, we are currently recruiting for several DFW territories. Flexible schedules available.

Responsibilities of this position include the following:

• Primary responsibility to manage incoming patient calls after hours and on weekends. Triage and visits as needed.
• Perform admissions and follow up visits.
• Coordinate with team members/physicians, supervising CNAs and participating in staff and Interdisciplinary Team meetings
• Assess patient’s physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DME
• Appropriately identifies/utilizes resources: Dietary, Pharmacy, Supplies, Home Medical Equipment, etc.
• Serves as a patient and family advocate; Maintains focus on the patient and family experience of care
• Completes documentation in a timely manner
• Provides education to patients, families, and caregivers
• Participates in patient care meetings as assigned
• Communicated effectively with coworkers and patients/family
• Participates in bereavement support
