QualificationsRequired:
- Minimum of an Associate degree.
- A track record of at least 2 years in hospice settings.
- Proficiency in one or more of the following: Electronic Medical Records (EMR), Other EMR systems, or TruChart EHR.
- Current Licensed Practical Nurse (LPN) license valid in Pennsylvania.
- Valid driver's license.
- Confirmation of previous discussions with a recruiter about this role.
- Willingness to undertake necessary travel.
- No prior applications or roles at this organization within the last six months.
- Submission of LPN License Number is advantageous.
Key Responsibilities
- Demonstrate proficiency in the use of tablets, mobile phones, and various EMR software.
- Manage time and personal schedule with a high degree of efficiency.
- Engage in collaborative patient care management and delivery.
- Adhere strictly to deadlines for medical charting.
- Provide comprehensive support to patients in designated areas.
- This position requires local travel.
