Lead LVN is responsible for the coordination of assessment, data collection, and submission of the Minimum Data Set ( MDS) & Payroll Based Journal ( PBJ). The position will be responsible for MDS, PBJ, and conversion & maintenance of paper medical records to electronic medical records. Assist with staffing of unit, staff evaluation, education of staff, other various duties assigned by the unit director.

Qualifications:Educational requirements: Minimum High School Diploma, GED, or equivalent. Graduated from a accredited LVN program. Possess a Valid CA LVN license, PALS and BLS certification, at least 2-3 years charge nurse experience required in a LTC, SNF, or subacute.
