Seeking either a full time or part time Nurse Practitioner to work with an established geriatric practice in Westchester County, NY. Geriatric certification is preferred however will consider an adult/family medicine trained clinician with geriatric experience. You will work with a board certified geriatrician to provide primary care services for the residents of assisted living facilities.

A full time hire will work 3-4 days per week in the field and 1-2 days working from home. You also will receive 4 weeks of paid time off.

A part time hire will work 1-2 days per week in the field.

Whether you are a full time or part time hire, you will be sent to the facility with a nurse to assist you in seeing your patients. The practice utilizes a very intuitive and user friendly EMR that is web based. You will be given a computer and a mobile WIFI to assist you in performing your visits.

Primary Responsibilities:

• Conduct assisted living facility visits of residents to function as their primary care provider. 
• Communicate with family members or HHAs in order to keep care givers up to date on patient needs and care plan.

• Follow up on diagnostic tests ordered such as blood work and radiology studies
• Order services such as visiting nurse and physical therapy.

Required Qualifications:

• Must have current New York State license as a Nurse Practitioner
• Must have experience in geriatric primary care
• Nurse Practitioner Certification from either the ANCC or AANP with Geriatric Certification or experience in geriatrics.
• Must be vaccinated against COVID-19
