Atrio Home Health is Hiring a Physical Therapist Near Grand Rapids, MI
Join Our Team as a Physical Therapist for Home Visits in the Grand Rapids Area with a $5,000 Sign-On Bonus Atrio Home Care, a partnership between Holland Home, Clark Retirement, and Resthaven home health agencies, is looking for a compassionate Physical Therapist to join our Grand Rapids Team.
This position involves providing physical therapy services to clients within a 35-mile radius of Grand Rapids.Responsibilities
Conduct initial physical therapy evaluations and ongoing assessments to determine functional level
Develop treatment plan for clients in consultation with the physician and other Atrio Home Health staff
Evaluates equipment needs to increase functional level and coordinates acquisition of needed durable medical equipment
Provide physical therapy treatment including gait training, transfer training, balance training, conditioning, and extremity strengthening
Identify barriers to desired outcomes and evaluates outcomes of care
Provide guidance to assigned Physical Therapy Assistants and Home Health Aides
Prepare clinical documentation, progress notes, and other required documentation
Participates in discharge planning with the Atrio Home Health team
Deliver clinical education to students
Address and resolve client and family concerns related to therapy
Location:
Grand Rapids, covering a 35-mile radius around GR
Shift:
Day shift 8-5pm, with flexibility in scheduling to help maintain a healthy work/life balance
Compensation
Our compensation model rewards our Physical Therapist for the work they do and offers bonuses to them for high-quality client outcomes and efficient case management.
Premium pay weekend and holiday visitsSign On Bonus
$2,500 bonus; paid in 3 increments over the course of your 1st year of employment
Qualifications
Physical Therapist License in the State of Michigan
Minimum of one year experience as a Physical Therapist; prior certified home health experience preferred
Geriatric experience is highly valued
Valid Michigan driver's license, auto insurance, and reliable means of transportation for home visits
