POSITION TITLE: Licensed Practical Nurse

DEPARTMENT: Med Surg

REPORTS TO: Med Surg Director

The Colorado Equal Pay for Equal Work Act requires employers in the state of Colorado to disclose the following information. Hourly (salary) minimum $21.00, hourly (salary) maximum $25.05. The base hourly (salary) range above represents the low and high end of the range for this position. Actual hourly (salary) rates may be above or below the range based on various factors including but not limited to: experience, education, and training.

Pay is just one component to prospective employees’ compensation package. Compensation may also include shift differentials. JOB PURPOSE: To improve the health and wellbeing of our communities.

BASIC FUNCTION: The Licensed Practical Nurse (LPN) provides care, treatment, counseling, and health education to swing bed patients. Provides professional nursing care to assigned inpatients. Monitors vital signs, administers medications within their scope of practice and develops swing bed patient care plans in conjunction with physicians and other members of the care team. Provides support to swing bed patients including bandage maintenance, pain management, and ensuring swing bed patients receive adequate rest, fluids, nutrients and planned activates required for swing bed patients. Observes swing bed patients’ progress, records observations in swing bed patient medical records, and updates attending Registered Nurses and physicians directly when necessary. Provides swing bed patient education and advocates for swing bed patients as necessary. The LPN assists in the maintenance of health and the prevention or management of illness, injury and disability. The LPN works as part of a health care team. Performs a variety of tasks and other duties as assigned.

QUALIFICATIONS:

Education: Must be a graduate from an approved program in practical nursing. Must possess a currently valid state of Colorado license to practice as an LPN. Must have current BLS card upon hire and maintain current status throughout GRHD employment.

Experience: Prefer one-year professional nursing experience in a healthcare setting.

KNOWLEDGE, SKILLS AND ABILITIES: Knowledge of the Licensed Practical Nursing theory and practice to give and evaluate patient care. Knowledge of organization policies, regulations and procedures to administer patient care. Knowledge of medical equipment and instruments to administer patient care. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in identifying problems and recommending solutions. Skill in preparing and maintaining records. Participation in developing and maintaining department quality assurance. Skill in establishing and maintaining effective working relationships with residents, medical staff, co-workers and the public. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to communicate clearly, both verbal and written. Must be proficient within six (6) months of hire in HealthStream, Kronos, and American Health Tech.

GENERAL COMPLIANCE REQUIREMENTS: Has knowledge of and adheres to all compliance regulations, policies and procedures. Completes annual compliance, privacy, and security training. Responsible for complying with all federal, state and local rules and regulations. Complies with the all Grand River Hospital District policies and procedures, the Code of Conduct and the Compliance and Ethics Program requirements. Reports any observation of fraud, waste, abuse, and/or privacy violations. Reports any conflict of interest or relationship immediately.

HEALTH INFORMATION PORTABILITY AND ACCOUNTABILITY ACT (HIPAA): Ensures and adheres to strict confidentiality when handling patient information, according to the HIPAA Privacy Act and hospital policy and procedure regarding confidentiality. Complies with all hospital information security practices.

WORKING CONDITIONS: While performing the duties of this job, the employee may be exposed to weather conditions prevalent at the time. This position performs tasks that involve Category I exposure to blood borne pathogens, body fluids or tissues. Exposure to patients’ conditions and some unpleasant sights, smells and diseases are common. Frequent exposure to chemical hazards and may encounter radiation hazards. May experience traumatic situations including occasional exposure to psychiatric, combative, dismembered or deceased patients.
