Job description

• Looking for Registered Dietitian to join our Skilled Nursing Home in the Queens area. - MUST BE LICENSED OR RD EXAM ELIGIBLE
• Accurately assess patient’s nutritional status, identify nutritional risk, develop plan of care and evaluate outcomes.
• Develop a written dietary plan of care that identifies the dietary problems/needs of the resident and the goals to be accomplished for each dietary problem/need identified.
• Participate in meal rounds to monitor intake and determine/address any issues related to food service and nutritional care.
• Participate in comprehensive care plan meetings with interdisciplinary team.
• Complete MDS accurately and in a timely manner.
• Review monthly weights and identify significant weight change and place appropriate interventions and documentation.
• Periodically assess adequacy of nutritional care provided to residents with feeding tubes and pressure ulcers.

Salary: From $40.00 per hour

Expected hours: 37.50 per week
