A DAY IN THE LIFE OF TRANSITIONAL CARE LIAISON (LPN)

As an LPN Transitional Care Liaison, you play an essential role in visiting patients in the facilities, physician offices, or patient homes to ensure they receive high-quality transitional care and education that meets their specific needs. When meeting with patients, you provide education, comfort, education, and at times hands-on nursing skills. You will also be responsible for education to facilities and physician offices regarding services and appropriate transitions to all levels of care. As a Transitional Care Liaison, you will be an integral part of the daily communication with facilities, physician offices, and patients. At times, you may be involved with transitional care conferences within multi-disciplinary networks. Transitional Care Liaisons will assist with DME, infusion, and other care needs. Through communication with other healthcare staff, you provide updates on each patient and clarify information when instructions are unclear.

To ensure the safety of your patients, you practice infection control and universal precautions. When needed, you report any forms of abuse and take appropriate action according to state and federal guidelines. Always reliable and punctual, you maintain regular attendance to complete your day-to-day tasks and submit any documentation regarding clinical notes and driving records. QUALIFICATIONS FOR A FIELD NURSE (LPN)

  • Active nursing license in the state of Oklahoma
  • CPR certification or the ability to obtain one within 60 days of being hired
  • 1 years of nursing experience
  • Valid driver's license and reliable transportation
  • Ability to travel and adapt to schedule changes
  • Basic knowledge about computers

Experience working in home health, critical care, emergency room, education, marketing, or transitional care would be a plus, but multiple factors will be taken into consideration. Do you have excellent verbal and written communication skills? Can you balance multiple tasks while demonstrating good time management? Are you able to easily adapt to new situations? Do you have exceptional problem-solving skills? Are you able to maintain a compassionate and empathetic manner toward others? If so, you might just be perfect for this nursing position!

WORK SCHEDULE

This full-time home health care position typically works from 8 AM - 5 PM, Monday to Friday.
