What you’ll do as a Home Health Speech Therapist/Language Pathologies (ST):

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge and understanding of individual development and human behavior as it relates to the effects of illness/injury; and of the influence of culture on health care.
• Knowledge of state and federal homecare regulatory guidelines preferred.
• Experience home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals is highly desired

• Prior to scheduling an in-person interview, all Candidates are required to complete a computer literacy and competency assessment. This is to ensure you are set up for success and can navigate in our environment including electronic charting, shared understanding of company values and use of our web based communication application. The results of the assessment are confidential and will be used as one of multiple data points included in the hiring decision process.

* You will be assigned to a Countyas a primary location, however you will be expected to be available to cover for vacancies, leave of absences, as well as supporting increased census and client care needs outside of the assigned area. 21st Century Home Health puts forth every effort to accommodate the assigned primary territory, but on occasion, in order to support safe patient care and meet the demands of a growing business, staff may be reassigned temporarily.
