Description
We are seeking an Integrated Clinic Associate to join our team! As an associate you are responsible for coordinating integrated medical, behavioral health and substance use care for clients. The Integrated Clinic Associate arranges regular multidisciplinary treatment plan meetings with the client, the treatment team, and other natural supports to create a person-centered plan of care. You will work collaboratively with the care coordination team and provides administrative support to the Psychiatry/Integrated Clinic programs.
Responsibilities
- Utilize patient registries to monitor population health data and required CCBHC activities; coordinate with treatment team and client to assist with completion and make referrals as needed.
- Administer social determinant of health screeners and coordinate with nursing/care coordination staff on follow-up for additional resources or services.
- Coordinate communication between the client, primary care, behavioral health, substance use, and other service providers, ensuring current releases of information are in place.
- Assist with scheduling hospital/emergency room follow-up appointments in psychiatry within 7 days of discharge and ensuring transportation/telehealth capability in place for the appointment.
- Facilitate and monitor internal and external referrals. Schedule internal referrals for Psychiatry and Integrated Clinic services.
- Track appointments and follow-up with the client if the appointment is missed.
- Assist with obtaining releases of information and other required forms.
- Prepare and administer required paperwork for client appointments including visit packets, screeners, surveys, consent forms, and others as requested.
- Coordinate with Integrated Clinic providers to manage client appointments and provider schedules including blocking group rooms, sending zoom invites, and providing appointment reminder calls.
- Act as Apple Tree Dental coordinator/liaison.
- Communicate non-CareLogic program in-person appointments to Support Services (Reiki/Apple Tree Dental).
Qualifications
Requirements and Qualifications
- High school graduate or equivalent.
- Minimum two years of experience in a behavioral healthcare setting with a focus on customer service/client engagement.

Pay: $22.15 - $26 per hour. Compensation is based on years of experience.
