What are the responsibilities and job description for the Case Manager position at Zoe's Way Culture?
Zoe Way Culture Youth and Family Behavioral Health Services
Location: Louisville, Kentucky Reports to: Clinical Director or designated qualified supervisor
Position Overview
Zoe Way Culture seeks an organized, resourceful Case Manager who can help youth and families obtain needed services and maintain continuity of care. This position coordinates practical supports across behavioral health, medical care, education, family services, and community resources for children and adolescents, including youth involved in foster care.
The Case Manager turns identified needs into coordinated action: clear service plans, timely referrals, completed connections, and documented follow-up. The role requires strong professional judgment, persistence, and the ability to work respectfully with youth, caregivers, providers, and community partners.
Essential Responsibilities
· Complete assigned case management assessments of strengths, needs, existing supports, and barriers to care. Gather information from youth, authorized caregivers, and relevant providers while respecting consent and confidentiality.
· Develop and update an individualized service coordination plan with the participant and care team. Identify priorities, measurable goals, responsible parties, and follow-up dates aligned with the clinical treatment plan.
· Connect families with appropriate medical, behavioral health, school, housing, food, benefits, transportation, and other community resources. Confirm whether referrals resulted in access and address unresolved barriers.
· Coordinate appointments, transitions, discharge follow-up, and changes in placement or services. Help participants prepare for meetings and understand next steps without making decisions on their behalf.
· Communicate with clinicians, peer staff, support staff, schools, legal guardians, foster caregivers, caseworkers, and outside providers as authorized. Clarify responsibilities and prevent conflicting or duplicated services.
· Monitor service participation and changing needs. Track missed connections, review progress with the team, and revise coordination activities when goals or circumstances change.
· Maintain timely, factual records of assessments, plans, contacts, referrals, follow-up, participant response, and outcomes. Keep consents and releases current and support record reviews.
· Manage an assigned caseload within applicable limits and service expectations. Keep an organized follow-up calendar, resource directory, and record of unresolved needs; promptly elevate barriers requiring leadership assistance.
Safety and Scope of Practice
Recognize and promptly escalate safety concerns to qualified clinical staff, follow emergency and reporting procedures, and coordinate follow-up. This role does not independently diagnose or provide psychotherapy. Staff assigned targeted case management serve only approved populations and do not provide other behavioral health services to the same client when prohibited by applicable requirements.
Required Qualifications
· Bachelor’s degree in social work, psychology, counseling, human services, family studies, or another qualifying behavioral science field from an accredited institution.
· At least one year of full-time experience after completing the degree, working directly with children, adolescents, or young adults in a human services setting. Experience coordinating services and working with families is required.
· For targeted case management assignments, meet Kentucky education, experience, training, and population-specific qualifications. Complete approved core and specialty training; assignments serving youth with a serious emotional disability (SED) require the 12-hour core and six-hour SED curriculum.
· Demonstrate skill in service planning, referral coordination, advocacy, prioritization, professional communication, and accurate electronic documentation. Understand consent, confidentiality, participant rights, and appropriate information sharing.
· Satisfy applicable background, registry, credential, and exclusion screening requirements. Complete credential verification and any required provider enrollment or payer affiliations through Zoe Way Culture.
Preferred Qualifications
Current Kentucky targeted case management certification for the assigned youth population; experience with Medicaid behavioral health services, foster care coordination, school collaboration, or wraparound services; and established knowledge of Louisville-area resources. An independent clinical license is not required for this position.
Supervision and Work Expectations
Work under a qualified supervisor and participate in case reviews. Targeted case management includes individual face-to-face supervision at least monthly during the first year and monthly group supervision throughout employment. Maintain required continuing education and renewal records. Additional qualifications and supervision must be verified before specialized complex-condition assignments.
Services may include scheduled community visits and team meetings. If driving is assigned, maintain a valid driver’s license and meet agency insurance and driving requirements. Transport participants only with authorization and required safeguards.
First 90 Days
· By day 30: Complete orientation and screening; verify qualifications; establish supervision; learn referral and documentation procedures; and build a working directory of priority community resources.
· By day 60: Complete assigned training and begin an approved caseload as readiness requirements are met. Develop individualized service plans, establish referral tracking, and demonstrate timely documentation and follow-up.
· By day 90: Complete credentialing with Zoe Way Culture and applicable enrollment requirements. Review the caseload with the supervisor, resolve overdue referrals, and report access barriers, completed connections, and participant progress.
Performance and Application
Success is measured through completed service connections, timely follow-up, current service plans, continuity during transitions, participant engagement, documentation quality, and manageable caseloads. Submit a resume, degree and training information, and professional references.
