A Primary Care Nurse Practitioner (PCNP) provides comprehensive healthcare services to patients of all ages. The PCNP works collaboratively with a multidisciplinary team to deliver high-quality, patient-centered care.
Key Responsibilities:
- Conduct thorough physical examinations and obtain medical histories.
- Order and interpret diagnostic tests (e.g., lab tests, x-rays).
- Diagnose acute and chronic conditions such as diabetes, hypertension, infections, and injuries.
- Develop and implement individualized treatment plans.
- Prescribe medications and other therapeutic treatments.
- Provide follow-up care and monitor patient progress.
- Educate patients and families on disease prevention, management, and health maintenance.
- Promote healthy lifestyles through patient education and counseling.
- Administer vaccinations and conduct routine screenings (e.g., blood pressure, cholesterol).
- Advocate for preventive care practices within the community.
- Work closely with physicians, nurses, and other healthcare professionals to coordinate care.
- Refer patients to specialists when necessary.

Qualifications:

- Experience: Minimum of 2 years of clinical experience in a primary care setting preferred.
